This refers to the NRC's Systematic Assessment of Licensee Performance

(SALP) 10 Report for the Davis-Besse Nuclear Power Station, our meeting of

September 16, 1993, in which we discussed in detail the contents of the
report, and your letter of respon:e dated September 23, 1993, relative to the

report.

Based on our discussions during the meeting and our review and evaluation.of

your letter of response, we have reached the conclusions presented in the

enclosed meeting summary. Several specific concerns were discussed, including
those involving personnel errors (particularly in the operations area),
control room noise levels, and contamination control problems encountered
during your most recent refueling outage. During our discussion, you and your
staff committed to certain followup actions which are also described in the
meeting summary.
If your understanding differs from that described, please
contact us immediately.

With the incorporation of the revised pages from Enclosure 3, the initial SALP.
