ROUTINE RADIATION PROTECTION INSPECTION AT LASALLE COUNTY STATION

This refers to the routine safety inspection conducted by Mr. P. L. Louden of
f'
this office on August 11 through 18, 1993. The inspection included a review
of authorized activities for your LaSalle facility. At the conclusion of the
t
inspection, the- findings were discussed with those members of your staff
3
identified in the enclosed _ report. A subsequent phone conversation was held
on September 1,1993, with members of your staff also identified within the
enclosed report.

Areas examined during the inspection are identified in the report. Within.
these areas, the inspection' consisted of a selective examination of procedures
and representative records, interviews with personnel, and observation of
activities in progress.
,
We informed you of your need for aggres'sive management actions with regards to
l
improving radiological housekeeping at your station in the cover letter which
.
forwarded NRC Inspection Report 50-373/93019(DRP);150-374/93019(DRP).
During
!
this inspection additional radiological concerns were identified that also
j
require aggressive management attention.
Specifically, your poor hose control
a
program resulted in the use of an underrated hose that failed during a tank to
l
tank waste transfer and caused a spread of contamination in the work area and
!
on personnel involved with the transfer.
Further, you have failed to ensure
g
,
that hoses were used for their appropriate applications whicn resulted in hose
cross contamination that precipitated the event described below.
l
During this inspection, certain of your activities appeared to be in violation
of NRC requirements, as specified in the enclosed Notice of Violation
(Notice). This violation involved an individual cutting and removing the sign
and ribbon used to post an area in which contaminated hoses were stored.
This
problem is of concern because the root cause of the violation was a willful
and deliberate action taken by an individual to remove the posting to gain
access to the controlled area.

This incident, while inconclusive as to the guilty individual, reflects a

careless disregard for radiological ~ controls. Willful violations will not be -
l
tolerated by the Commission, and this action was reviewed for Escalated
- l
Enforcement. -The factors weighed during our review to determine an
.
appropriate Severity Level included; the position and responsibilities of
individuals which may have performed the action; the intent of the violator;
and any economic or other advantage that may have been gained by the action.

After reviewing these factors, we have determined that no supervisory
individuals were involved with the event, the intent did not involve
!
!
disturbing or gaining access- to safety-related equipment, and no personal gain.

not be considered for the event.
You are required to respond to this'1etter and should follow the instructions
specified in the enclosed Notice when preparing your response.
In your
response, you should document the specific actions taken and any additional
actions you plan to prevent recurrence. Your response should also include the

corrective actions'you plan to take to ensure there.is adequate oversight of-
l
the hose control program at your facility. After reviewing your response to
,
this Notice, including your proposed corrective actions and the results of
'
future inspections, the NRC will determine whether further NRC enforcement
action is necessary to ensure compliance with NRC regulatory requirements.
In accordance with 10 CFR 2.790 of the Commission's regulations, a' copy of

this letter, the enclosures, and your response to this letter will be placed
j
in the NRC Public Document Room.
The response directed by this letter and the accompanying Notice are not
subject to the clearance procedures of the Office of Management and Budget as

required by the Paperwork Reduction Act of 1980, PL 96-511.
