This refers to the routine announced inspection conducted by Messrs. C. Clark
and M. Payne, of this office, July 19 through July 23, 1993 and August 2

through August 6,1993, and by Mr. J. Colaccino of the office of the Nuclear
Reactor Regulation / Mechanical Engineering Branch on July 19 through July

23, 1993, of activities authorized by NRC Operating License Nos. NPF-10 and
NPF-15. The results of this inspection were provided in discussions held by
the inspectors with Mr. L. Cash and other members of your staff on July 23

and August 6, 1993.

Areas examined during this inspection are described in the enclosed
j
inspection report. Within these areas, the inspection consisted of selective
j
examinations of procedures and representative records, interviews with
j
r
personnel, and observations by the inspectors.
Based on the results of the inspection, it appears that one of your
activities was not conducted in full compliance with NRC requirements, as set
forth in the enclosed Notice of Violation (Notice). The violation involved
examples of failure by your inservice testing personnel to completely
,
document the performance of surveillance testing of safety related
I
components.

You are required to respond to this letter and should follow the instructions
specified in the enclosed Notice when preparing your response.
In your
response, you should document the specific actions taken to prevent
repetitive violations and any additional actions you plan to prevent
j
l
recurrence. After reviewing your response to this Notice, including your
proposed corrective actions and the results of future inspections, the NRC
l
will determine whether further NRC enforcement action is necessary to ensure
compliance with NRC regulatory requirements.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of
this letter and its enclosures will be placed in the NRC Public Document
Room.
