SUMMARY

During the period of May 22,1993 to June 15,1993 Eddy Current Testing (ECT) and mechanical plugging were performed on all four steam generators (SG's) in accordance 1

with Technical Specification 4.4.5. Other SG primary side activities this outage included a visual inspection of all tube plugs / PAP's installed to date, plug repairs on all cold leg Westinghouse mechanical plugs made from Heat No. NX-3513 using Babcock and Wilcox Plug a Plug (PAP) device, and tubes were repaired by roll expansion (RX).

The results of each inspection and repair conducted during the End of-Cycle 17 refueling outage relative to the steam generators is summarized in Section 1.2 thru 1.5.

1.1 Definitions The following terms are used in this report:

1.1.1 ATS-Above Tubesheet - Used to identify an area where flaws are located.

I 1.1.2 ATSP - Above Tube Support Plate. Used to identify an area where flaws are located. In this report ATSP flaws are included in one group. There are 4 tube support plates in the Westinghouse Series 27 SG.

1.1.3 AVB - Antivibration Bar 1.1.4 Defect - In accordance with Connecticut Yankee Technical Specifications, any flaw that shows 50% or greater thruwall. Also, any flaw located less than 1" below the roll transition.

l 1.1.5 ECI-Eddy Current Testing i

i 1.1.6 E* - A minimum required amount of sound roll needed between the

bottom of the roll transition and the top of a crack in the expanded region of the tube in the tubesheet. At Connecticut Yankee the F* length is 1".

1.1.7 Flaw - Any indication that shows a percent thruwall 2 20%.

1.1.9 S_Q - Steam Generator 1.1.10 1%E - Babcock & Wilcox designed " Plug a Plug" used for mechanical plug repair.

A 100% full length tube ECT inspection was performed on each steam generator (13,987 total tubes').

-j 1.2.1 The inspection revealed that 3,042 tube sides contained one or more reportable flaws (= 22% of total tubes tested).

78% of the flaws were located above the tubesheet,13% were located above the tube support plates and 6% at the anti vibration bars. The remaining flaws were located within the tubesheet, mostly in the hot leg :

I roll transition region.

1.2.2 As a special test, U-bends of approximately 159 lower row tubes and 94 dented tube support locations were examined with a motorized rotating pancake coil (MRPC) probe for PWSCC. No tubes were found to contain PWSCC.

1.3 Mechanical Plugging A total of 39 tubes were plugged during the 1993 refueling outage. After 17 cycles of operation,8% of the steam generator tubes have been plugged.

1.3.1 Completion of the 1993 refueling outage plugging effort brings the total

i SG #1 247 SG #2 414 SG #3 108 SG #4 459 1.4 Visual Inspection A visual inspection for leakage of all previously installed plugs and plugs repaired with leak limiting retainer devices was performed. ' All items inspected were found j

to be acceptable.

- 1.5 Mechanical Plug Repair (PAP)

J Potentially defective cold leg Westinghouse mechanical plugs from HT #3513 were j

repaired by Connecticut Yankee by installing plug repair inserts which are i

manufactured by B&W. The B&W plug-a-plug (PAP) inserts are designed to prevent a plug top release incident, capture potential loose parts, and reduce leakage through defective plugs to acceptable levels.

l A total of 590 PAP's were installed in Westinghouse mechanical cold leg plugs produced from'the susceptible Inconel 60011 eat #3513.

.j 1.6 Tube Repair (Roll Expansion)

A roll expansion (RX) process was used to repair defective steam generator tubes which contain PWSCC in the roll transition region of the tubes. Roll expansion is a 1

process whereby a new mechanical joint is produced by hard rolling the tube against the tubesheet above the existing tube defects. The defective portion of the tube is removed from the primary pressure boundary and the repaired tube is l

acceptable for continued service.

l A total of 83 tubes were repaired by the roll expansion (PJ) process.

d 3.0 DISCUSSION 3.1 ECT Inspection i

3.1.1 Table 1 provides a summary of the entire SG ECT Inspection workscope performed during the 1993 Refueling Outage.

3.1.2 A 100% full length tube ECT inspection was performed in all four SG's (13,987 tubes). In addition, tubes with PWSCC within the tubesheet roll region were inspected with the specially designed F* probe. Tubes identified by bobbin coil ECT to have possible roll transition flaws were inspected with a 3-coil motorized rotating pancake coil (MRPC) probe.

During the 1986 ECT program, PWSCC was identified within the roll expansion region of several SG tubes in all four steam generators. The roll expansion region is a 4.25 inch section at the end of each tube that is expanded against the tubesheet on both the inlet and outlet side of the SG. The cracks tended to be oriented in the axial direction and occurred i

t primarily at regions of tube geometry variations ('i.e., roll transitions).

3.1.3 The Row I U-bends (61 tubes)in SG 4 and Row 2 U-bends (98 tubes) in SG 1 were inspected with a flexible U-bend MRPC probe and did not identify any U-bend cracking.

3.1.4 This outage also included an inspection of dented locations at tube supports using a gimbled MRPC probe. This probe,in sizes small enough to traverse large dents, is a surface riding MRPC probe designed to identify flaws within dented tube support k) cations. A total of 94 dented tube support locations were inspected with no flaws identified.

i 3.1.5 The inspection this outage did, however, identify some large percent i

throughwall defects (with bobbin) at orjust above the hot leg tubesheet in SG 1. A MRPC inspection was performed on these flaws and indicated i

that some of these tubes contained circumferential oriented indications approximately 1 inch above the hot leg tubesheet. An expanded MRPC inspection workscope was conducted, which identified a total of 11 tubes J

in SG 1,4 tubes in SG 2, I tube in SG 3 and 2 tubes in SG 4 containing circumferential oriented indications.

Ultrasonic tests (UT) were performed on 10 of the 18 tubes containing circumferential oriented indications and confirmed the existence of shallow O.D. degradation. The results of the ultrasonic testing showed the degradation to be 30% TW and made up of several short segments.

The most probably degradation mechanism is shallow wastage with tightly adhering O.D. deposits. An assessment of these indications concluded that in no case were the requirements of Regulatory Guide 1.121 exceeded.

3.1.6 The results of the 1993 inspection showed 3,042 tube sides out of 13,987 tubes tested (22%) contained reponable flaws.

3.2.1 A total of 39 tubes were plugged during the 1993 refueling outage.

Completion of the 1993 steam generator effon brings the total number of tubes plugged to 1228.

3.3 VisualInspection.

A visual inspection of all previously installed plugs and plugs repaimd with leak limiting retainer devices (i.e., B&W PAP's) was performed to supplement other inspections. For plugs and PAP's, the only rejectable condition is one in which water is observed to be actively dripping under static secondaty side head or when the secondary side is pmssunzed. All plugs and plugs with leak limiting retainers that were examined in each SG were found to be acceptable.

2 3.4 A total of 590 plug retainer devices, termed PAP's, were installed this outage in specific cold leg plugs locations. These plug repairs complete the scope of repairs required to address plug failures associated with PWSCC of susceptible Westinghouse plugs produced of Inconel 600 Heat #3513.

3.5 Tube Repair by Roll Expansion i

A new r.,Jr technique utilizing Roll Expansion (RX) was implemented, for the first time, during the outage. The use of the roll expansion process is to repair defective steam generator tubes which contain pnmary water stress corTosion -

cracking (PWSCC) defects in the hard roll region. The tube repair by RX is a process where a new mechanical joint is created by hard miling the tube against the tubesheet above existing tube defects. The defective portion of the tube is removed from the primary pressure boundary and a new F* boundary is established, thereby allowing the tube to remain in service.

A total of 83 tubes in two steam genemtors were repaired suing this method during the outage.

4.0 SUMM ARY OF STEAM GENERATOR EXAMINATION RESULTS summarizes the location and maximum pement throughwall depth per tube j

side fnr various flaw types identified during the 1993 eddy current exammation.

The secondary side flaws included in the attachment represent each of the four secondary i

side defect mechanisms (ATS pitting / wastage, ATSP pitting, AVB wear, and ATS cire indications). This method of summarizing the inspection results provides a means of comparing the various defect mechanisms.
