SUBJECT: REQUALIFICATION EXAMINATION REPORT NO. 50-334/93-19 (OL-RQ)

During the week of August 23,1993, the NRC administered requalification examinations to

10 employees of your company who operate the Beaver Valley 1 facility. The examination

addressed areas important to public health and safety and was conducted in accordance with

Revision 7 to NUREG-1021, " Operator Licensing Examiner Standards," ES-601 through  !

604. As 16 individuals were observed during the simulator portion of the examination, an -

l evaluation of your requalification program was made. Preliminary results were discussed at

l an exit meeting held on August 26 with Mr. Chatfield, Mr. Freeland, and members of your  ;

'

j staff identified in the attachment to this letter.

The examination of the operators was conducted to determine if they would satisfy the

regulatory requirement of passing an NRC-administered examination prior to license renewal.

As evaluated by the NRC, all ten operators examined passed all portions of the  !

requalification examination. The four crews, comprised of 16 individuals, were evaluated as .

satisfactory on the simulator portion of the examination. As more than 12 individuals were

examined on the simulator and due to the 100% pass rate, we determined that your

'

requalification program retained its satisfactory rating.

During the simulator crew evaluation, your staff determined that one individual's

performance did not meet Duquesne Light Company's standards for operator performance.

The individual was a staff licer. sed Senior Reactor Operator, who performed during the

simulator examination only. We understand that a remediation plan is being prepared to

address this individual's performance deficiencies. Our assessment is that your program

acted properly in identifying the individual's performance problems and in developing an

appropriate remediation plan to address these problems.
