Dear Mr. Feigenbaum:
SUBJECT: NRC INSPECTION REPORT NO. 50-443/93-13
This letter transmits the report of the safety inspection findings of the resident staff at
Seabrook Station for the period of June 15 through July 26,1993. The inspection consisted
of document reviews, personnel interviews, and observations of activities important to the
health and safety of the public. The inspector discussed the findings with Mr. W. DiProfio
and members of your staff after the inspection.
Overall, this inspection found that activities were conducted safely. The inspection identified
one violation of regulatory requirements which involved two different instances where plant
workers did not follow procedural instructions. Each personnel error resulted in a challenge
to plant equipment. The violation is cited in the enclosed Notice of Violation. You are
required to respond to this letter and should follow the instructions specified in the enclosed
Notice when preparing your response. In your response, you should document the specific
actions taken and any additional actions you plan to prevent recurrence.
These personnel errors are similar in nature to others that occurred during the previous
routine inspection period as documented in NRC Inspection Report No. 50-443/93-10, dated
June 23,1993. Although each event had a low safety consequence, in the aggregate, the
personnel errors appear to require a comprehensive approach to reduce the frequency and
signiGeance of the errors. Based on your presentations at our management meeting at NRC
Region I on August 20, I understand and appreciate that you are now applying substantial
management attention to this issue. After you have responded in writing to the Notice of
Violation, I propose that a followup management meeting be held in the Region I ofGce or at
the site. At this meeting, please be prepared to discuss (1) the significance of the personnel
errors, (2) your initial failure to assure early detection of the declining performance, and
(3) your corrective action plan to improve performance.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter and
its enclosures will be placed in the NRC Public Document Room.
