INITIAL' SYSTEMATIC ASSESSMENT OF LICENSEE PERFORMANCE (SALP) REPORT l This forwards the initial SALP report (50-445/93-99; 50-446/93-99) for the

Comanche Peak Steam Electric Station. The SALP Board met on June-23, 1993, to l i evaluate Comanche Peak's performance for the period of February 3, 1992, through May- 29,'1993. The performance analyses and_resulting evaluations are

documented in-the enclosed initial SALP report. In accordance with NRC: policy, I have reviewed th'e'SALP Board's assessment and concur with their ratings as discussed below: Good performance in the areas of Plant Operations and Maintenance / i e l- Surveillance resulted-in a Category 2 rating. -Although some improvement in the area of missed surveillances and operator performance was noted, errors in system configuration control, other personnel errors, and missed-surveillances continue _to require increased management attention. l e Performance remained high in the area of Radiological Controls and a Category I rating was assigned. but a declining trend was identified due

                     ' primarily. to instances of ineffective comunications and poor radiological work practices _during stressful times _such as the refueling outage.-

! Jo Performance declined in the Security area and a Category 2 rating was L assigned. Managerial- problems were noted with respect to oversight of i the contract. security organization and to supervision within~certain security _ shift' organizations. I

e. A decline in performance in the Safety Assessment / Quality Verification functional area resulted in a Category 2 rating. As mentioned in the i ' prev 1cus-SALP report, strong management involvement continued to be p evicent in this area,.but you have not been totally effective in L 1mplementing corrective actions _ or in responding aggressively to deficiencies identified by your self-assessment organizations.

TU Electric ' e Performance in the areas of Emergency Preparedness and Engineering / Technical Support continued to be superior, resulting in Category I ratings. e Superior performance in the areas of Preoperational Testing / Operational Readiness and Power Ascension Testing for Unit 2 resulted in Category I ratings. Excellent use of lessons learned from Unit 1; strong programs for room, area, and system turnover; and a cautious, deliberate approach - in the execution of startup tests contributed to these ratings. Overall, safety performance at Comanche Peak was good, and improvements were noted in certain programs. Particularly noteworthy was the concurrent completion of preoperational testing on Unit 2 while undertaking a refueling outage on Unit 1. Also noteworthy was the continued strong performance in the areas of Emergency Preparedness and Engineering / Technical Support, which reflected strong management involvement and safety perspective. A decline in performance was noted in some functional areas and ongoing problems were noted in others. Strong management support for plant activities was evident, but weaknesses in the implementation of certain programs and a failure at times to ensure that performance expectations were being met contributed to the decline. In order to prevent a further decline in performance, additional management attention is appropriate. In particular, I , encourage you to consider the following actions: (1) continue to emphasize i the implementation of the corrective actions initiated to enhance personnel performance and further reduce personnel errors in the areas of operations and surveillance testing, (2) provide increased management oversight of the contractor guard force, and (3) strengthen the implementation of the i corrective action process and the response to self-identified issues. I also emphasize the need for your continued sensitivity to problems that may ' surface as you fully integrate the activities of two operating units. You should assure that the transition to an operating plant environment is successfully achieved. It is essential you ensure appropriate engineering support of plant operations. In accordance with NRC policy for recently licensed plants, the next SALP period will be 12 months. Accordingly, the next SALP period will be from May 30, 1993, to May 28, 1994. A management-meeting has been scheduled with you and your staff to review the  ! results of the initial SALP report. The meeting will be open to the public ' and held at the Personnel Processing Center at the Comanche Peak site on August 13. 1993, at 10 a.m. Within 20 days of this management meeting, you may provide comments on ana amplification of, as appropriate, this initial SALP report.
