Dear Mr. Broughton:
SUBJECT: NRC INSPECTION REPORT NO. 50-289/93-14
This letter transmits the findings of the resident safety inspection conducted by Ms. M. Evans
and Mr. D. Beaulieu from June 22,1993 - July 31,1993, at the Three Mile Nuclear Station.
The inspectors based these findings on observations of activities, interviews, and document
reviews and discussed them with Mr. M. J. Ross and other members of your staff. Inspector
observations indicated safe and conservative operation.
Based on the results of this inspection, one of your activities appeared to be in violation of
NRC requirements, as specified in the enclosed Notice of Violation (Notice). Due to an
inadequate surveillance procedure, the bolts for the 'B' emergency diesel generator (EDG)
lubricating oil filter cover were not properly torqued. As a result, the ability of the EDG to
continue to perform if called upon in an emergency was uncertain, because a repair under
very difficult conditions would have been required. You are required to respond to this letter
and should follow the instructions specified in the enclosed Notice when preparing your
response.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter and
its enclosures will be placed in the NRC Public Documec Room. The responses directed by
this letter and the enclosed Notice are not subject to the clearance procedures of the Office of
Management and Budget as required by the Paperwork Reduction Act of 1980, Public Law
No. 96.511.
