SUBJECT: NOTICE OF VIOLATION (VERMONT YANKEE INSPECTION 93-13) )

This refers to the safety inspection conducted by Messrs. H. Eichenholz and P. Harris on ]'
May 23 - June 26,1993, at the Vermont Yankee Nuclear Power Station, Vernon, Vermont.
The preliminary results were discussed with Mr. R. Wanczyk at the conclusion of the
inspection. The inspection focused on issues important to public health and safety, and
consisted of performance observations of the conduct of operations and maintenance, and
independent evaluation of safety system status and quality records.
Our observations this period were that the plant was operated safely. Corrective actions for
a leak rate test deficiency of containment sampling piping were effective and enforcement
discretion was therefore exercised. However, your failure to properly calibrate the core
spray sparger differential pressure instruments resulted in a violation of Technical
Specification requirements, as specified in the enclosed Notice of Violation. We are
concerned that this finding exhibited weaknesses with your assessment of industry experience
and biennial procedure reviews, which represented missed opportunities since 1979 to
identify the problem, Nonetheless, we do acknowledge the excellent performmce on the part
of an Auxiliary Operator whose questioning attitude identified this condition, and we
encourage your actions to continue to identify and correct such problems.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practices," a copy of this letter
and the enclosed Notice will be placed in the NRC Public Document Room. You are
reo,uired to respond to this letter and should follow the instructions specified in the enclosed
Notice. The responses directed by this letter and the enclosed Notice are not subject to the l
clearance procedures of the Office of Management and Budget as required by the Paperwork l
Reduction Act of 1980, Public Law No. 96.511. l
