tion to those otherwise required, of the Shipper's Export Dectoration covering each export ond mork one of such copies for transmittol to the U.S. Nuclear Regv.
lotory Commission, Washington, D.C. 20555. The following decloration should oc-company or be placed on the Shipper's Export Declarations for such exports:
"This shipment is being mode pursvont to specific license nurnber (specific license number) filed at (locotion of Customs office where license is filed),
on (dote license was filed). This license expires on (expirotion date of license), and the unshipped bolonce remaining on this license is sufficient to cover the shipment described on this dectoration."
Condition 2 - Exports authoriaed in any country or destinction, except Country Groups Q, S, W, X, Y, and Z in Port 370, Supplement No.1, of the Comprehens ve Export Schedule of the U.S. Department of Commerce.
Condition 3 - This beense covers only the nuclear content of the materiol.
Condition 4 - The materiol to be exported under this hcense shall be shipped in accordonce with the physical protection requirements for special nuclear materiol in 10 CFR 73.
Condition S - Special nuclear material outhorized for export under th;s Fcense shall not be transported outside the Umfed States in possenger carrying circraft in shipments exceeding (1) 20 grams or 20 curies, whichever is less, of plutonium or vronium 233, or (2) 350 groms of uranium 235.
Condition 6-This license authorires export only and does not authorire the rece;pt, phys' col possession, or use of the nuclear material.
Condition 7 - The licensee shall complete and submit on NRC Form 741 for each shipment of source material exported under this license.
Condition 8 - The licensee shall advise the NRC in the event there is any change in the designo.
j tion of the company who will pockoge the nuclear material to be exported under i
this bcense, or any change in the locotion of the pockoging operation, of least three weeks prior to the scheduled dote of export.
Condition 9 - "The material to be exported under this license shall be protected in transit, while within U.S. jurisdiction, either in accordance l
with the requirements of 10 CFR Part 73 and the licensee's or his I
agent's approved security plan or by the Department of Energy (DOE) i Safe Secure Transport (SST) system in accordance with DOE requirements and directives for the transport of such material."
