HIGHLIGHTS OF WEEKLY INFORMATION REPORT WEEK ENDING JUNE 8, 1990
Indian Point 2 The licensee performed the staff requested visual inspection of the lower interior surf aces of SG #24 by using a TV camera lowered into the annulus between the tube bundle wrapper and the steam generator shell.
The results of this inspection were recorded on video tape and reviewed by NRC staff personnel. Based on its review of this video tape, the staff concurred with the licensee's conclusion that the lower portions of the steam generator shell were essentially free of surface pitting. Therefore, the licensee is planning to begin heatup of the plant on approximately June 10, 1990 and expects to synchronize the generator to the grid on June 16, 1990.
A grey of visitors from the Soviet Union visited Indian Point 2 on June b. 1990. The visitors are members of Working Group 2 of the US USSR Joint Coordinating Comittee on Civilian Nuclear Reactor Safety. The purpose of the site visit is to acquaint the visitors with a U.S.-designed PWR facility.
The Comittee to Review Generic Requirements is planning to visit Indian f
Point Units 2 and 3 on August 1-3, 1990. This is expected to be a routine visit during which the Cornittee plans to tour the plant, including the EOF, and meet with utility management and an operating crew.

Peaceful Uses Agreement

On June 1 Presidents Bush and Gorbachev signed a five-year agreement *on Scientific and Technical Cooperation in the Field of Peaceful Uses of Atomic This was initialed by V. Konovalov, USSR Minister of Nuclear Power Energy."
and Industry, and John Easton, U.S. Assistant Secretary of Energy (International),
on May 25. The Agreement supersedes a similar Agreement which had been in force since 1973. Chairman Carr attended the signing ceremony at the White House as a guest of Secretary of Energy Watkins.
Cooperation in nuclear reactor safety, administered by NRC, is one of the four components of the Agreement. The substance of this program will be unchanged from that now in effect.
(The other three components of the Agree-ment cover nuclear research administered by 00E.)
An NRC Team Visit to the URENCO Enrichment Plants During the week of June 4, 1990, an NRC team of safeguards, safety and security specialists visited three Euro)ean uranium enrichment plants operated by URENCO Ltd. The three plants are: tie Almelo Plant in the Netherlands, the Capenhurst Plant in Great Britain and the Gronau Plant in West Germany.
The purpose of the visit was to identify the safeguards and safety concerns related to operation of centrifuge enrichment plants. This information is needed in the development of the licensing requirements and guidance for the proposed Louisiana Energy Services (LES) enrichment plant which is to be built in Homer, Louisiana.
The LES plant will be the nation's first privately-owned facility for providing nuclear fuel enrichment services.
Cintichem, Tuxedo, NY On May 30, 1990, a meeting regarding the decommissioning of Cintichem facilities was held with the New York (NY) Department of Labor. Understandings reached at the meeting form the basis for drafting an agreement on what should be included in the decommissioning, decontamination criteria to be used by the licensee, and arrangements for and conduct of verification surveys.
NRC attendees included representatives from the Office of Nuclear Material Safety and Safeguards, Office of Nuclear Reactor Regu'ation, and Region I.
New York was represented by the State k partments of Labor, Health, and Environmental Conservation.

Grand Gulf Unit 1; Waterford 3; Arkansas Nuclear One, Units 1 and 2 Entergy Corporatiti advised the staff on June 6, 1990, that the Securities and Exchange Commission (SEC) approved the consolidation of operation management for Grand Gulf, Unit 1. Waterford 3, and Arkansas Nuclear One, Units 1 and 2 under a single company, Entergy Operating, Inc. (E01). The staff had previously reviewed and approved the consolidation by issuing license amendments subject to official implementation by June 12, 1990.
Approval by the SEC and public service commissions was necessary before official implementation.
Entergy Operations will also serve as licensing agent for the Grand Gulf, Unit 2 Construction Permit, which is expected to be officially withdrawn this year. A letter from Entergy Operations is expected shortly to make the implementation official. The staff in NRR has consolidated licensing management of these units under Project Directorate IV-1.
Meetina With FAA On May 31, 1990, members of the Division of Reactor Inspection and Safeguards met with an Aviation Safety Inspector (Manufacturing), FAA to discuss NRC's policies and procedures for identifying, investigating and disseminating information on counterfeit / fraudulent parts supplied by vendors to the nuclear industry.
FAA is in the process of setting up a system to accomplish the some purpose for FAA activities.

Office of Nuclear flaterial Safety and Safeguards Items of Interest Week Ending June 8, 1990 SAFEGUARDS AND TRANSPORTATI0ft International International Atomic Energy Agency (IAEA) Physical Inventory Verificatio Lat the Babcock & Wilcox (B&W) Fuel Company On June 11, 1990, an International Safeguards Branch staff member will observe the physical inventory at the B&W Fuel Company in Lynchburg, VA.
The staff member will also be present when the IAEA conducts their physical inventory verification starting on June 14, 1990.

Transportation Meeting on Westinghouse Titan Cask On June S, 1990, Transportation Branch staff met with Westinghou."
sectric Corporation (Westinghouse) and the Department of Energy (DOE) to discuss the preliminary design of the Titan spent fuel shipping cask. The Titan cask is being designed under contract to DOE as a legal weight truck cask for shipments of spent fuel under the Nuclear Waste Policy Act.
The cask has a capacity i
l of four pressurized water reactor or seven boiling water reactor assemblies.
I Issues discussed included material properties and welding procedures for fabricating the cask body from titanium and efforts to incorporate titanium alloy in Section III of the AS!!E Code.
An application for NRC certification is expected in January 1993.

Office of Nuclear Regulatory Research Items of Interest Week Ending June 8, 1990 Division of Enaineering National Research Council Panel on Coupled Systems The National Research Council's panel on the effects of coupled hydrologic, tectonic, and hydrothermal systems at Yucca Mountain, Nevada met at the U.S.
Geological Survey in Menlo Park, California, on May 30-31, 1990. This was the second meeting of *.he newly formed panel.
The suitability of Yucca Mountain for High-Level Waste (Hl.W) disposal is being evaluated by the Department of Energy. Dr. Edward O'Donnell represented the Waste Management Branch, Office of Nuclear Regulatory Research (RES) at this panel meeting which focused on the i
hydrologic system.
There were technical prest.ntations by the U.S. Geological Survey, the State of Nevada and its contractors, and the Department of Energy (DOE). Some of the topics discussed included the regional hydrogeology, the local hydrogeologic framework of the Yucca liountain area, stability of the Yucca Mountain area hydraulic regime in responso to underground nuclear explosions, paleoclimates of the region, effects of past pluvial climates on the regional water table, and parameter variability in the hydrologic modeling of Yucca Mountain.
The meeting concluded with a presentation by Jerczy Szysmanski, DOE, on his concept of a regional coupled model and its long-term effects. The next National Research Councti panel meeting will be held July 26-k7 to examine the geologic / seismic / tectonic system.

Office for Analysis and Evaluation of Operational Data Items of Interest Week Ending June 8, 1990 Division of Operational Assessment (DOA)
The Zion Diagnostic Evaluation Team arrived at the Zion Station on Monday, June 4, to begin two weeks of onsite evaluation activities.
The entrance meeting for the team took place at 1:00 p.m., the first day onsite. The meeting was attended by the AEOD Deputy Director, Region III Administrator and Ceco Corporate and Senier Station Management representatives.
The " Response Technical llanual" (RMT-90), NUREG-0150. Volum",s 1 and 2, was published by IRB this week.
It contains the response tcchnical procedures used by the Protective Measures and Reactor Safety Team members.
This document is the foundation of the Protective Measures Workshop which is going to be held on June 18-22, 1990 at the Federal Emergency Management Agency's (FEMA) National Emergency Training Center. Each regional office received copies for use by the AE0D staff during the upcoming Fall training sessions on this subject matter.
On June 6, IRB staff attended a meeting of the Catastrophic Disaster Response Grtup Subcommittee on Federal Earthquake Response to discuss catastrophic response planning, forthcoming exercises, and new energy emergency support functions.
On June 4 IRB staff attended an Interagency meeting to discuss the comments of all federal agencies pertaining to revisions of the Environmental Protection Agency's Orotective Action Guides.
Additional meetings have been planned to resolve the issues raised by the comments.
Division of Safety Programs (DSP)
AEOD made a presentation to ACRS on the Accident Sequence Precursor (ASP) program status and results.
The presentation focused on significant insights obtained from the ASP program from 1984 to 1988.
The First Quarter of the Quarterly Performance Indicator report was issued on June 7, 1990.

Catawba Emergency Planning Exercise Richard Woodruffe Region II State Agreements Officer, participated in an emergency planning exercise at the Catawba Nuclear Station on June 7 in York, South Carolina. Mr. Woodruff served as Government liaison on the site team.
Election to American Academy of Health Physics Joel Luber.au, State Programs, has been elected to the office of Treasurer for the American Academy of Health Physics. itr. Lubenau's term of office will begin at the Winter Meeting of the Academy in 1991.

JCCCNRS Working Group Meetings On June 2, a Soviet delegation arrived in Washington for two JCCCNRS Working Group meetings.
One Working Group covers "the analysis of the safety of nuclear power plants" (WG-2) and the other covers " diagnostics, analysis equipment and systems for supporting operators" (WG-9).

Two Millionth NUDOCS Record The two millionth record was coded into NUDOCS last week. This milestone will be recognized by the presentation of a plaque to the individual who entered the record.

Vital Records Program The Information and Records Management Branch (IRMB) staff visited the Federal Emergency Management Agency (FEMA) emergency relocation site and updated the vital records inventory.
IRM has been advised by the AEOD Emergency Response Coordinator that five additional emergency relocation sites will be used to serve the five NRC Regional Offices. AEOD has expressed an interest in IRM supporting these five new emergency relocation sites by providing and maintaining selected vital records. At this time, it is unclear which records may be required or the method that will be used to maintain the records at these sites.
AEOD will provide additional information to IRM about their plans for the new relocation sites.

l Office of Personnel items of Interest Week of June 8,1990 Emoloyee Assistance Procram Conference Held Pat Kaplan, Employee Assistance Program Officer (Training, Development and Employee Assistance), attended a one day conference on June 7, 1990, at the Psychiatric Institute of Montgomery County in Rockville, Maryland.
The conference covered new approaches in mental health care, the increasingly important role of employee assistance programs in the work environment, and trends in care for the elderly.
Small Acency Council Personnel Automation Conference On June 5, Lillian van Santen (Training, Development and Employee Assistance) attended a conference at the National Science Foundation in Washington, D.C.
The conference provided a hands-on demonstration of their Training Request Tracking System.

Office of Small and Disadvantaged Business Utilization and Civil Rights Items of Interest Week Ending June 8, 1990 Civil Rights Program-The Office of Small and Disadvantaged Business Utilization and Civil Rights hosted the agency's Sixth Annual EE0 Counselor Training Conference May 22-25, 1990 in Hunt Valley, Maryland..
. Conferees, totaling thirty, included Regional and Headquarters EE0 Counselors and a representative from each of NRC's EE0 Advisory Committees. The Conference began with opening remarks from the Director, OSDBU/CR, followed by a presentation from Mr. Douglas Bielan.from the Equal Employnent Opportunity Commission.
EE0 Counselor training was provided by Creative Conmunications-Associates, a small business contractor. Caro?yn Swanson, Carol Harris and Barbara Williams.from the Office of Personnel also contributed to the Conference by providing invaluable information on matters ranging from staffing to labor relations.

Off. ice of Enforcement Items of Interest Week Ending June 8, 1990 Significant Enforcement Actions 1.
An Order Imposing a Civil Penalty in the amount of $6,750 was issued June 6, 1990 to Tulsa Gamma Ray, Tulsa, Oklahoma.
This action was based on violatiens that, in the aggregate, were considered to represent a breakdown in control of licensed activities.
The violationt, occurred in the following programmatic areas:
- 1) job-site radiation safety
- 2) personnel radiation exposure evaluation and recordkeeping; 3) practices; radio-graphy device inventories; and 4) transportation of radiography devices.
The licensee responded to the Notice in a letter dated February 22, 1990.
After careful consideration of the licensee's response, the staff has concluded that nine of the ten violations did occur as set forth in the Notice of Violation and Proposed Imposition of Civil Penalty. The original proposed civil penalty has been reduced by 107., because the licensee submitted additional information to show that one of the ten violations did not occur. (EN 89-116A) 2.
A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $160,000 was issued June 7,1990 to Minnesota Mining and ihnu-facturing Company.
This action was based on seven violations relating to 3M's failure to test and evaluate the use of static eliminators by general licensecs, failure to make the required notifications to NRC and to these general licensees, and making a design modification to three models of static eliminator without prior NRC review and approval. The staff submitted this action for the Connission's review in SECY-90-172 which was approved by the Commission.

Bass !!emoriti Baptist Hospital, Enid Oklahoma paid the civil penalty in the amount of $3,125. The action was based on the NRC's identification of 26 violations related to the licensee's failure to: comply with NRC's requirements related to ensuring the proper operation of its dose cali-brator; perform surveys and calibrate survey instruments; inventorying labeling and maintaining surveillance over licensed materials; and keep required and adequate records required by(the radiation safety program.
The civil penalty was escelated by 25%.

U.S. Testing Company, Modesto, California paid the civil penalty in the l
amount of $5,000.
The action was based on a number of violations of NRC requirements designed to protect members of the public against exposure in excess of Part 20 limits. The violations included the failure to properly establish and provide surveillance of radiation boundaries during radio-graphic operations at a hospital.

Tennessee Valley Authority (Browns Ferry) paid the civil penalty in the amount of $18,750.
The action was based on the licensee's failure to conduct an adequate annual inventory of licensed material which resulted in the discovery, in a later inventory, of 26 discrepancies. Although this violation was classified at Severity Level IV, the licensee had been cited on two previous occasions for similar violations.

Nine Mile Point Two Region I safeguards inspectors are participating in a Regulatory l
Effectiveness Review (RER) at Nine Mile Point Nuclear Power Plant during the period June 4-8, 1990. Other team participants include representa-I tives from NRR/ Reactor Safeguards Branch and members of the U.S. Army.
The purpose of the RER is to review the effectiveness of the safeguards systems and determine if the Nine Mile Point security program, as implemented, is meeting the performance objectives of 10 CFR Part 73.
The review focuses particular attention on the methods and procedures employed to protect equipment vital for the safe shutdown of the reactor.

On June 5. Comissioner Remick and members of his staff, accompanied by the Projects Division Director and the Resident Inspectors toured the Surry Nuclear Power Station.

On June 7, a full-scale emergency exercise was conducted at tne Duke Power Company's Catawba facility.

Carolina Power & Licht Company June 8, the Regional Administrator met with CP&L management representatives at their Corporate Office in Raleigh, North Carolina regarding CP&L's nuclear program self-assessment in response to the recent requalification examination results.

Clinton Nuclear Power Station On June 5, 1990, the Deputy Regional Administrator and other Region III staff conducted an Enforcement Conference on site with senior licensee management to discuss several inadequacies identified in the Shutdown Service Water (SX) system. Original design-and testing problems resulted in substantial deficiencies in service water flow rates to critical safety related components.
In addition, during followup to discovery of this problem the licensee valved out service water flow to both diesel generators during reactor operation.
The licensee's engineering evaluation of these matters is continuing.
The regional staff will be proposing an enforcement action.
On Ju, : 5, 1990, the Director, Division of Reactor Safety, presented senior and reactor operator licenses at an award dinner in Decatur, Illinois.
On June C, 1990, the Deputy Regional Administrator accompanied Commissioner Curtiss on a visit to the Clinton Power Stction.

Nuclear Power Stations From June 5, 1990, chrough June 7, 1990, a delegation representing a Taiwan utility com9any toured emergency preparedness (EP) facilities and discussed EP programs with Comonwealth Edison Company representatives. The three-member delegation was accompanied by l
representatives from the Illinois Department of Nuclear Safety. They visited Dresden Station, the Emergency Operations Facility (EOF) in liazon, Illinois and the Braidwood Station.

Commonwealth Edison Company at the Dresden and Quad Cities sites, respectively, to tour tie plants and discuss items of mutual interest with the licensee.

Kewaunee Nuclear Power Plant On May 31, 1990, a management meeting was held between licensee representatives and members of the NRC headquarters and Region III staffs.
Topics of discussion were changes to the Kewaunee management organization, changes to the security program, engineering and engineering technical support projects, and their plans for steam generator tube recovery during the 1991 outage.

Gulf' States Utilities On Wednesday, June 6, 1990, Gulf States Utilities reported that one of the two duplicate comauter systems used to activate the River Bend Station sirens failed-to worc during a periodic siren activation test. When the sirens failed to activate, the licensee immediately switched to the alternate computer and a silent test was successfully accomplished.

requiring sounding the alarms was performed because the public had been i
previously advised of the exact time of the scheduled test, and the licensee did not want to cause public concern. The licensee notified FEf1A, the state of Louisiana, snd the affected Parishes.
