Dear Sir:
Action Item 3 of your letter, Reference (b), requested information for certain closures and connections in the Reactor Coolant Pressure Boundary.
The following information is provided, as directed by Action Item 5 of your letter, Reference (b).
QUESTION 3a:
Identify those bolted closures of the RCPB that have experienced leakage, particularly those locations where leakage occurred during the most recent plant operating cycle. Describe the inspections made and corrective measures taken to eliminate the problem.
If the leakage was attributed to gasket failure or its desig1, so indicate.

Mr. Ronald C. Haynes, Regional Administrator Page two RESPONSE: Valve Ebdy to Bonnet Leakage 05/05/75 - LSI-12 and 32 (ANSI Class 1500 lb 10 in. Swing Check Valves *);
changed cover gaskets because of leakage.
03/10/82 - SlA-N-ll ( ASA 1500 lb 14 in. Motor Operated Gate Valve *); found evidence of leakage, tightened gasket bolting, no further leaks.
03/27/82 - LSI-22 ( ANSI Class 1500 lb 10 in. Swing meck Valve *);
tightened gasket bolting, no further leaks.
- Leakage was on closures separated from the Reactor Coolant System by a closed check valve.
Steam Generator Manway Leakage Steam Generator No. 1:
03/07/80 - Evidence of leakage noted when shutdown; replaced gasket.
Steam Generator No. 2:
10/02/74 - Leaked during hydro (following opening); replaced gasket.
05/02/77 - Leaked during hydro (following opening); replaced gasket.
07/07/81 - Leaked during ISI Pressure Leak Test (following opening);
replaced gasket.
10/25/81 - 3/8/82 - Slight leakage noted. Leakage cu? to improper design of seal plate preventing adequate gasket compression. Seal plates modified to correct problem.
Reactor Vessel Flange March 1982 - Leakage past both 0-ring seals.
Seals failed due to fouled seating surface.
Surface cleaned and new seals installed.
QJESTION 3b:
Identify those closures and connections, if any, where fastener lubricants and injection sealant materials have been or are being used and report on plant experience with their application particularly any instances of SCC of fasteners.
Include types and composition of materials used.
RESPONSE: Fastener Lubricants and Injection Sealant Materials Maintenance practices require the use of approved lubricants when reassembling bolted closures.
As currently specified by procedure, Fels Pro N-5000 is the approved bolting lubricant (Super Moly No. 40240 was used for the Reactor Vessel Closure Studs, however, its use has been discontinued).

Reference (c) and subsequent correspondence provide further information on the Steam Generator Manway problem.
Further information required by Reference (b) will be provided as specified by Action Item 4.
