14-Day Follow-up Reports LER 82-024 and 82-025, Dated June 23, 1982 On June 23, 1982 the referenced correspondence transmitted the 14-Day Follow-up Reports and LER's82-024 and 82-025 concerning two inadvertent violations of the Technical Specifications 3.0.4 and 3.7.1.2 Action b.

In that letter, we stated that an investigation to determine the cause of the oversight in failing to recognize in a timely manner the full impact of the Work Order on valve 2HV4713 and AFW System operability was continuing. We also stated that any corrective actions resulting from this investigation would be identified by July 9,1982.

We have completed our investigation and this submittal provides that information.

The oversight resulted from inadequate review of the Maintenance Work Order'(W.O.) by the SRO. The W.O. did not provide sufficient information to serve as a valid basis for the SRO's decision not to declare the AFW train associated with Pump P-141 inoperable. He did not recognize this and hence the error.

Mr. R. H. Engelken July 9, 1982 We had recognized the need for a Program' revision before the incident and had initiated action to write and issue new procedure 5023-0-26,

" Equipment Deficiencies". This procedure describes and clarifies the Operations Department implementation of its responsibilities associated with Maintenance activity. When issued, this new procedure will result i in clearer documentation on Work Orders of problems to be corrected,  !

verification of these descriptions by knowledgeable individuals, and l more complete reviews for Technical Specifications LCO impact. The new l procedure wii! be issued by August 27, 1982. '
