9 APPENDIX A NOTICE OF VIOLATION

'4ashington Public Power Supply System Docket No. 50-508 P. O. Box 1223 Elma,llashington 98541-As a result of the inspection conducted on June 7-11, 1982 and in accordance with the NRC Enforcement Policy,10 CFR Part 2. Appendix C, 47 FR 9887 (March 9,1982), the following violation was identified:

10 CFR 50, Appendix B, Criterion X and paragraph 17.1.10 of the Quality Assurance Program in Section 17 of the PSAR state in part: "... Examinations, measurements, or tests of material or products processed.shall be performed for each work operation where necessary to assure quality...."

Contract specification No. 3240-224, paragraph 3.1.4.3 states in part: "...The penetration assemblies shall be installed, inspected and tested in accordance with ASME Boiler and Pressure Vessel Code, .

Section III, Subsection NE for Class MC. Components..."

The ASME Boiler and Pressure Vessel Code 1977 Edition including addenda through Winter 1979 Section III, Subsection NE, Paragraph NE5221 applies the requirements of NE5211.1 to category B weld joints. Paragraph NES211.1 states in part: "Where the joint detail does not pemit radiographic examination, ultrasonic-examination plus liquid penetrant or magnetic particle examination may be substituted for...the closing seam within an electrical penetration assembly...."

Contrary to the above requirements on June 8,1982 neither magnetic particle nor liquid penetrant examinations were specified or performed on field weld flo.1 for each of electrical penetration flos. 102, 104 and 106. Field weld No.1 is a category B butt welded joint foming a portion of the containment pressure boundary. lleld braze data cards for these joints had been reviewed and approved by contractor personnel and the authorized nuclear inspector and had been completed and returned to quality assurance records.

This is a Severity Level IV Violation (Supplement II).
