i APPENDIX A GO'11CE OF VIOLATION 4

Au a result of the inspection conducted en May 1-28, 1982 and in accordance with the DRC Enforcement Policy, 10 CFR Part 2 Appendix C, 47 FR 9687 (March 9,1962), the following violation was identified:
10 CFR 50 Appendix B Criterion V states, " Activities affecting quality shall be prescribco by documented instructions, procedures, or draw-ings, of a type appropriate to the circumstances and shall be accom-plished in accordance with these instructions, procedures, or drawings.
i instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."
This is addressed in the WhP-1 Quality Assurance Program description in Section 17 paragraph 17.4.5 of the PSAR.
The electrical contractor's quality assurance manual, Chapter QAF-13 part 6.4, requires that, "Where special precautions are required for handling of items to prevent damage or other considerations, detailed instructions and/or procedures shall be prepared and implemented."
The contractor's implementing instructions included the following requirements:
a.
Westinghouse installation procedure PEN-TR-48 which details the installation of electrical penetration assemblics, in-cluding the installation of cable trays into the penetration sleev.3a prior to insertion of the cables, b.
Foley-Wismer-Lecker procedure QCP-7 part 4.7 which requires a process traveller for owner-furnished equipment, including hold points for quality inspection.
c.
Foley-Kismer-Eccker procedure QCP-10 which defines cable pulling criteria, including inspection for safe-edges and jacket condition.
Contrary to the above, on February 3, 1982 the electrical contractor (Foley-Uismer-Lecker) subcontracted the installation of the electrical penetration assemblics to the containment steel contractor (Pittsburgh-DesMoine.a Corporation) without inclusion of appropriate installation procedures, including quality control inspection requirements for the electrical cable handling.
As a result, the following hardware discrepancies were identified on May 21, 1982, relating to installa-tion activities conducted during the period of March 2 through May 21, 1982:

In each case the cables had been installed into the penetration sleeves prior to insertion of the cable trays, inconsistent with the work sequence set forth in the beatinghouse instructions.
The cable trays contained soue sharp edges, such that the forcing of the trays into the sleeves under the heavy cable bundles could have resulted in abrasion and damage to the cable insulation jackets.
b.
ihe completed installations of some cables, (including those foc penetrations lil!4 and !!J4) resulted in the heavy cable bundles resting on the sharp edges of the pipe sleeves at the inside containment wall, with potential for long-term damage to the cable insulation jackets.
(This matter has since been corrected by lacement of cushion material under each such cable bundle.
This is a Severity 1cvel V Violation (Supplement II), applicabic to Unit 1.
Pursuant to the provisions of 10 CFR 2.201, Washingtcn Public Power bupply Systeu is hereby required to submit to this office within thirty days of the date of this notice, a written statecent or explanation in reply, including:
(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further items of noncompliance; and (3) the unte when full compliance vill be achieved.
Consideration may be given to extending your response time for good cause shown.
