Dear Mr. O'Reilly:
Reportable Occurrence Report R0-369/82-48 was to have been submitted by July 5, 1982. This 30 day reportable occurrence (per T.S.6.9.1.13) concerns T.S.3.4.5, "All power operated relief valves (PORVs) and their associated block valves shall be operable"; and T.S.3.3.3.6, "The accident monitoring instrumentation channels shown in Table 3.3-10 shall be operable", which were violated on June 5, 1982. At this time, investigation of the incident has not been completed. The report will be submitted by July 16, 1982.
