Docket No. 50-206 Special Report Regarding Fire Detection Surveillance San Onofre Nuclear Gewrating Station, Unit No. 1 Pursuant to Technical Specification Section 6.9.3.c, this is a Special Report as defined in Technical Specification Paragraph 3.14.8.(4).b concerning Fire Detection Instrumentation.
On May 17, 1982 it was identified that the semi-annual Zone 3 Fire Detection surveillances, which include the 4160V Switchgear Room Halon Fire Suppression Actuation System Test and the smoke detector channel functional tests, were not completed during July, 1981 as required by Technical Speci fication 4.15.D.(1).
The required surveillances on the other Fire Detection Zones were completed between July 23-27, 1981; however, the maintenance surveillance j
procedure in effect at the time did not contain sufficient guidance to disable l
the 4160V Switchgear Room Halon Fire Suppression System in order to perform the Zone 3 smoke detector functional tests and the Halon Fire Suppression Actuation System Test.
In order to perform the Zone 3 Fire Detection surveillances, procedural revisions and Halon System modifications were found to be necessary.
The system was modified in late 1981 and the smoke detectors functionally tested on January 18, 1982.
The modifications which permitted the performance of the smoke detector surveillances were found at that time to be insufficient to complete the semi-annual 4160V Switchgear Room Halon Fire Suppression Actuation System Test.
An alternate means of accomplishing the Halon System Actuation Test employing a different method and procedure was utilized to complete this portion of the Technical Specification requirements on March 24, 1982.

U.S. Nuclear Regulatory Commission June 16, 1982 A specific corrective step already implemented to address this situation includes a tracking system for all Technical Specification-related maintenance orders.
This system assigns a "due date" and a " late date" for such maintenance orders.
If the maintenance order is not closed on or before the "due date", the Maintenance Department initiates an investigation as well as appropriate corrective action to ensure maintenance order completion by the
" late date".
Corrective steps which will be taken to preclude the recurrence of incidents of this type include:
a.
Maintenance Procedure S01-I-2.22, "6 Month Fire Detection Channel Functional Test", will be revised to include the Halon System test methods identified in Maintenance Procedure 501-I-2.24, "18 Month Halon System Functional Test".
The revision to S01-I-2.22 will be implemented by July 16, 1982.
b.
A briefing for appropriate Unit 1 Maintenance Department personnel on this incident, including compliance with Technical Specification requirements, will be completed by July 1, 1982.
c.
The Unit 1 Maintenance Department will conduct a review of all other Fire Protection Systems surveillances to ensure that requirements have been met.
This review, which will be performed in conjunction with the review commitments made in our April 23.
1982 Special Report on Spray / Sprinkler Systems and in our response to the Notice of Violation issued with IE Inspection Report 50-206/82-15, will be concluded by July 16, 1982.
Should any other deficiencies in the performance of Technical Specification surveillances be uncovered, a formal report will be submitted.
d.
Maintenance Procedure S01-I-1.4, " Maintenance Order Preparation, Use, and Scheduling", is in the final stages of preparation and will be issued by August 2, 1962.
This procedure incorporates the use of a Maintenance Order Tracking Log and will require Quality Assurance, Engineering, and Supervisor of Plant Maintenance review of each maintenance order before it is assigned to a Maintenance Foreman.
