Dear Mr. Keppler:

On May 25, 1982, Mr. S. J. Brewer of Public Service Company of Indiana, Inc. (PSI) notified your office of a potentially reportable item as required by 10 CFR 50.55(c). During an audit at Brown-Boveri Electric, Inc. (BBC) facilities PSI quality assurance personnel dis-covered that welding techniques not covered by BBC's quality assurance program were being used in the fabrication of safety related switchgear.

Such activities are special processes and should have been controlled by procedure as required by 10 CFR 50, Appendix B, Criterion IX. It was also discovered that no controls were placed on the terminal lug crimping process. Finally, BBC could not present certification for its circuit breaker wiring. With these deficiencies it is possible that installed equipment could differ from that ussd for IEEE 323/344 qualification.

PSI has issued a stop work order on BBC facilities until this problem l

is resolved. BBC has also been directed to respond to a Corrective Action Request which requires them to address the cause of the problem, correct-ive action, and quality of items affected prior to discovery of the prob-lem.

This letter is intended to fulfill the requirements of an interim report as required by 10 CFR 50.55(e). A further report on this item will be filed on or before October 29, 1982. If you have any questions on this report, please feel free to contact me.
