This refers to the special inspection conducted by Messrs H. Canter and J.
O'Brien of this office on February 11 through April 28, 1982, of circumstances associated with the inoperability of an energency diesel generator and a high pressure injection (llPI) pump at the Rancho Seco plant in February and March 19S2.
The results of the inspection were discussed by Mr. Canter with Mr. Rodriguez and other members of your staff on flarch 10, 1982. The results were also discussed at an enforcement conference on April 21, 1982, with you and other members of your staff. We are concerned that your management control system pennitted inoperable safety-related equipment to be returned to service due to inadequate implementation of safety-related procedures.
In addition, proper implementation of your routine shift surveillance practices should have detected these conditions during the first shift following the return of the safety-related equipment to service. As you are aware, these deficiencies led to a llPI limiting condition for operation (LCO) being violated on March 1,1982, and the diesel generator LC0 being violated on February 11, 1982.
The significance of these violations is increased due to their repetitive nature. Specifically, Licensee Event Report (LER) 79-11, dated September 13, 1979, described the six-day inoperability of Reactor Building Emergency Cooling Unit A-500C. This event was the result of a mispositioned toggle switch for a circuit breaker charging spring motor. Additionally, escalated enforcement was taken when the NRC issued a civil penalty and orver because safety-related equipment (high prassure injection pump) was not properly returned to service following maintenance. floreover, LER 81-33, dated July 3,1981 concerned an inoperable diesel due to unwareness of the status of the fault flags which is similar to Item 1 of the Appendix.
