APPENDIX A Stone and Webster Engineering Corporation Docket No. 99900509/82-01 NOTICE OF NONCONFORMANCE Based on the results of an NRC inspection conducted on March 8-12 and March 22-26, 1982, it appears that certain of your activities were not conducted in accordance with NRC requirements as indicated below:

Criterion V of Appendix B to 10 CFR Part 50 states:

" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.

A.

Engineering Department Memorandum No. 82-3 dated January 29, 1982, stated that Experience Records that have not been prepared or updated within one year must be prepared or updated by March 1, 1982, (commit-ment to the NRC).

Contrary to the above, all reguired experience records for designers in the electrical and the engineering mechanics division (Boston office) were not prepared or updated by March 1, 1982, as committed to the NRC.

Engineering Assurance Procedure (EAP) 5.3 - Preparation and Control of Manual and Computerized Calculations (Nuclear Projects) states in part:

... Review of the assumptions shall be based upon the reviewer's experience and knowledge and shall assure affirmative answers to the l

Are all assumptions that recuire confirmation at a later date specifically identifiec as assumptions that must be l

confirmed?...

Corporation Contrary to the above, an assumption made in calculation No.12179-US(B)-221, " Geometric Input Nodalization for the Auxiliary Building" stated that all areas sealed with weather-stripped doors are not exposed to high energy line breaks from adjacent areas.

This assumption was not identified as an assuoption that must be confirmed and there was no confirmation made to assure that the doors specified would withstand the pressures generated in adjacent compartments.

The specification for a door at one location did not address any pressure requirements.

C.

EAP 5.3, " Preparation and Control of Manual and Computerized Calculations (Nuclear Projects)," states that checking "Yes" on the calculation title page indicates the calculation contains inputs / assumptions that require confirmation at a later date.

Contrary to the above, HVAC calculation PB-196, for the control building cooling load, contained input information that was not confirmed and "Yes was not checked to indicate that confirmation was required at a later date.

The inputs noted that required later confirmation were:

(1) General Electric supplied information that was identified as preliminary; and (2) heat load information for computer equipment was provided by memo without knowing the final configuration or supplier of the computer.
