r APPENDIX A Westinghouse Electric Corporation Docket No. 99900217/82-01 NOTICE OF NONCONFORMANCE Based on the results of an NRC inspection conducted on April 19-22, 1982, it appears that certain of your activities were not conducted in accordance with NRC requirements as indicated below:

Criterion V of Appendix B to 10 CFR Part 50 states:

" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accom-plished in accordance with these instructions, procedures, or drawings.

Instructions, procedures, or drawings shall include appropriate quantita-tive or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."

Nonconformances with these requirements are as follows:

A.

Product Assurance Manual, dated June 12, 1981, section 9.0, states in part:

" Control of special processes is accomplished in accordance with established Quality Assurance Procedures."

Contrary to the above, no Quality Assurance Procedures were established which addresses the qualification of the wire wrap process and personnel.

B.

Quality Assurance Procedure 10.2, Revision

, Certification of Amp Termi point Wiring Personnel, section 2.1, states:

"All... personnel assigned to perform or inspect Termi point wiring shall be certified in accordance with the requirements of this procedure."

Contrary to the above, one of the two operators performing Termi point wiring of cabinet assembly 1065E58G02, Serial Number 0002 on April 20, 1982, was found to not be certified in accordance with the procedure.

No records could be found to demonstrate that the required training and testing had been accomplished.

Quality Assurance Procedure 13.3, Revision B, Process Specifications -

Use and Control, section 2.2.1 states in part:

" Process Specifications

.. will be maintained at Inspection Stations."

Contrary to the above, although wire wrap was observed on several boards in the PC Board assa, the applicable Process Specification-(PS 82355 HA) was not maintained at the Inspection Station.

The specification identified the acceptance criteria to be used by inspection personnel.

D.

Quality Assurance Procedure 14.6, Request for Engineering Action, Revision A, section 3.1.1.1.(9) states in regard to completing Requests for Engineering Action (REA):

... enter the equipment serial number when one has been assigned."

Contrary to the above, REA's 342565 and 342566 were observed on serialized equipment which did not have the assigned serial number entered on the REA's.

Quality Assurance Procedure 14.1, Inspection Control Tag, Revision A, section 2 states in part: "The Inspection Control Tag is normally used to control... assemblies.

The control tag provides a permanent record of inspection / test status and configuration level of the product."

Contrary to the above the procedure was found to be not fully implemented to control assemblies in that:

The requirement of paragraph 3.1.1.3.13 of the procedure, pertaining to updating of tags, was not accomplished for Job. No. 10477, part 1064E03, as of April 21, 1982, to reflect change order, RN 3088, which had been issued on March 23, 1982.

2.

Recording of all operator's signatures and dates, which is a require-ment of paragraph 3.2.1.1 of the procedure, was not accomplished on the tags for parts 1063E43G41, SN0001, and 1063E43G39, SN0001, for Job No. 10477.

3.

Shortages (i.e., missing serial number stamping and missing bracket, and three power supplies) were not identified on the tag, as required by paragraph 3.2.1.4 of the procedure, for part 1063E43G39, SN0001 for Job No.10477.

4.

A Defective Article Report (DAR) number was not entered on the tags for part 6090072G01, SN0005 for Job No. 10531, although required by paragraph 3.3.1.6.1 of the procedure.

It was also noted that two nonapplicable REA's had been recorded on the tags.
