FINAL REPORT - DER 82-12 DEFICIENCY EVALUATION 50.55(e)

ARIZONA PUBLIC SERVICE COMPANY (APS)

PVNGS UNITS 1,2 & 3 I.

Description of Deficiency The Controls Discipline Field Engineer review of installation documents in preparation for completing N-5 Code Data Reports discovered that the 3D Clamps securing the instrumentation lines were not installed in ac-cordance with the requirements of installation specification 13-JM-702 Appendix B.

These requirements provide for thermal expansion of the tubing.

II.

Analysis of Safety Implications This condition is evaluated as reportable since if not detected and repaired it could possibly result in loss of essential instrumentation.

III.

Corrective Action Nonconformance Report JX-512 will be dispositioned to review and rework all discrepancies to the requirements of the installation specification.

The individuals responsible for these installations have reviewed this report to preclude recurrence.

The installation specification will be clarified by issuance of a specification change notice 3019 to provide additional guidance for installing instrument tubing to compensate for thermal expansion.
