APPENDIX A NOTICE OF VIOLATION Tennessee Valley Authority Docket Nos. 50-438 & 439 Bellefonte 1 & 2 License Nos. CPPR 122 & 123

As a result of the inspection conducted on March 1-31, 1982, and in Accordance with the NRC Enforcement Policy, 47 FR 9967 (March 9,1982), the following violation was identified.
10 CFR 50, Appendix B, Criterion IX and Tennessee Valley Authority (TVA)
Final Safety Analysis Report Section 17.1A.9 requires that "lieasures shall '
be established to assure that special processes, including welding, heat s
treating, and nondestructive testing, are controlled and accomplished by qualified personnel using qualified procedures in accordance with applicable codes, standards, specifications, criteria, and other special requirements."
Bellefonte's Quality Control Procedure BNP-QCP-10.13, Rev. 5 requires that all welding shall be performed using qualified detail weld procedures.
Contrary to the above, on liarch 9,1982, a welder informed the resident inspector that he had re-melted three pipe welds in the Boric Acid Pump Room without a detail weld procedure.
This is a Severity Level IV Violation (Supplement II.D.).
Pursuant to the provisions of 10 CFR'2.201, you are hereby required to submit to this office within thirty days of the~ date of this Notice, a written statement or explanation in reply, including:
(1) admission or denial of the alleged viola-tion; (2) the reasons for the violation if admitted; (3) the corrective steps which have been taken and the results achieved; (4) corrective. steps which will be taken to avoid further violations; and (5) the date when full compliance will be achieved.
Consideration may be given ~to extending your response time for good cause shown.
