t APPENDIX A NOTICE OF VIOLATION Mississippi Power & Light Company Docket No. 50-416 Grand Gulf 1 License No. CPPR-118 As a result of the inspection conducted on March 15 - April 8,1982, and in accordance with the NRC Enforcement Policy, 47 FR 9987 (llarch 9,1982), the following violation was identified.

10 CFR 50, Appendix B Criterion V as implemented by MP&L Operations Quality Assurance Program, section 5.5, requires that activities affecting quality or safety be prescribed in instructions, procedures, or drawings and be per-formed in accordance with the instructions, procedures or drawings. Grand Gulf Startup thnual Chapter 5000, paragraph 4.1.4.8, requires data takers to sign data sheets and to correct errors with a line through and initial.

Paragraph 4.1.5.1 requires a test change notice (TCN) where a test is changed and paragraph 4.2.6 requires the Nuclear Plant Manager to accept test procedure results. Chapter 7000, paragraph 4.2.2, requires the test supervisior to sign for completion.

Contrary to the above, on April 1,1982,1E30PT01, Suppression Paul Makeup Preoperational test, contained three unsigned data sheets, paragraph 7.3 was not signed by the test supervisor, data sheet 8.6 contained corrections which were not lined through and initialed and the results were accepted by the Nuclear Support ihnager. IE21PT01, Low Pressure Core Spray Preoperational test, startup form had Instrument 1E21-K150 calibration deleted without a test change notice,1M51PT01, Drywell Cooling Preoperational test had corrections made to paragraphs 7.3.10, 7.3.11 and 7.3.102 without a line through and initial and paragraphs 7.3.102 and 7.3.110 had operating conditions changed without a test change notice.

This is a Severity Level V Violation (Supplement II.).

Pursuant to the provisions of 10 CFR 2.201, you are hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including: (1) admission or denial of the alleged viola-tion; (2) the reasons for the violation if admitted; (3) the corrective steps which have been taken and the results achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date when full compliance will be achieved. Consideration may be given to extending your response time for good cause shown.
