ARIZONA PUBLIC SERVICE COMPANY (APS)
PVNGS UNIT 3 1.
Description of Deficiency During a Bechtel Field Engineering review of placement records for Pour 3F006, a total of eight (8) - #8 horizontal bars were found to have been omitted between Elevation 97'-9" and 100'-0" in the Fuel Building as per Drawing 13-C-ZFS-100, Revision 12.
This condition is attributed to oversignt since this area, the Fuel Transfer Tube Inspection Station, is a small projection attached to the exterior wall of the Fuel Building, and the missing rebar were inadvertently overlooked prior to concrete placement.
II.
Analysis of Safety Implications This condition is evaluated as Not Reportable. This condition if not detected and corrected could possibly result in cracking and minor structural failure in a localized area of the inspection station during an SSE.
This postulated event, however, is evalu-ated as not safety significant. This affected area is designed for shielding only and is not an essential structure.
III.
Corrective Action Nonconformance Report CF-3316 will be dispositioned to chip concrete, install eight (8) horizontal rebars as per interim NCR disposition and pour-back this area with the next concrete placement. The inspection will now use the next elevation placement, which mare clearly indicates the horizontal tie bars.
All of the individuals responsible for pre-placement inspections have reviewed the report to preclude recurrence.
