Appendix NOTICE OF VIOLATION The Detroit Edison Company Docket No. 50-341 As a result of the inspection conducted on January 1 through February 28, 1982, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violations were identified:

1.

10 CFR 50, Appendix B, Criterion XVI, states in part, " Measures shall be established to assure that conditions adverse to quality, such as failures...are promptly identified and corrected..."

The Detroit Edison Fermi 2 Quality Assurance Manual, Section 17.1.3, t

l states in part, "In the case of significant conditions adverse to quality, Quality Assurance shall assure that the cause of the con-ditions is determined, and that corrective action is taken to preclude repetition."

Contrary to the above, inadequate corrective actions were taken with regard to the failure of the core spray system expansion bellows during system hydrotest, in that actual pressure and displacement conditions which contributed to failure of the bellows were not factored into the evaluation of the failure, and were not documented and reported in accordance with the licensee's corrective action system.

This is a Severity Level IV violation (Supplement II).

10 CFR 50, Appendix B, Criterion V, states in part that " Activities affecting quality shall be prescribed by documented instructions, procedures...and shall be accomplished in accordance with these instructions, procedures..."

The Detroit Edison Fermi 2 Quality Assurance Manual, Section 9.1.5, states in part, " Activities related to the performance of quality assurance functions affect quality and shall be performed...in accordance with prescribed instructions, procedures or drawings."

Contrary to the above, on January 21, 1982, the inspector found that:

RCI Procedure AC-1, Revision 2, requirements regarding access cleanliness control were not being implemented in the area of the reactor pressure vessel (RPV), in that a clean room had not been established for entry material and tools were not being logged in and out of the RPV, and personnel were allowed to enter the RPV without removing or securing loose personal articles.

The licensee's subcontractor failed to provide documented instructions for an activity affecting quality, i.e., the removal of machining chips from the control rod drive housing.

This is a Severity Level V violation (Supplement II).

10 CFR 50, Appendix B, Criterion XVI, states in part, " Measures shall be established to assure that conditions adverse to quality are promptly identified and corrected. The identification of the significant con-dition adverse to quality...shall be documented and reported to the appropriate levels of management."

The Detroit Edison Fermi 2 Quality Assurance Manual, Section 17.1.1, states in part, " Quality Assurance and Quality Control personnel shall promptly identify and report on conditions adverse to quality..."

Contrary to the above, on January 21, 1982, the licensee's subcontrac-tor's quality control personnel failed to promptly identify and report on machining chips found in the control rod drive housings in accordance with the licensee's procedures.

This is a Severity Level V violation (Supplement II).

10 CFR 50, Appendix B, Criterion XV, states in part that " Measures shall be established to control materials, parts or components which do not i

conform to requirements in order to prevent their inadvertent use or installation."

The Quality Assurance Manual, Section 7.0.1, states " Written procedures shall be implemented which delineate the methods and responsibilities

l for assuring that material, equipment and services procured by Edison, or by its vendors and contractors, conform to the requirements of the procurement documents."

Contrary to the above, effective measures were not established to prevent the installation of nonconforming control rod drive housings (i.e., they contained machining chips) in the reactor pressure vessel.

This is a Severity Level V violation (Supplement II).

Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written state-i ment or explanation in reply, including for each item of noncompliance:

(1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full com-pliance will be achieved. Under the authority of Section 182 of the Atomic l
