I APPENDIX A NOTICE OF VIOLATION Public Service Electric and Gas Company Docket / License: 50-354/CPPR-120 Hope Creek Generating Station, Unit 1 During inspection on April 5 - May 2,1932, and in accordance with the HRC Enforce-ment Policy,10 CFR 2. Appendix C (47FR9987, March 9,1982), the following viola-tions were identified:

10 CFR 50, Appendix B Criterion XIV states, in part, that: 'Teasures shall be established to indicate, by the use of markings...the status of inspec-tions...upon individual items of the nuclear power plant... These measures shall provide for the identification of items which have satisfactorily passed required inspections...where necessary to preclude inadvertent by-passing of such inspections..."
Section 16.2.15 of Chapter 16 of the PSAR states, in part, that: " Control of the use of inspection and welding stamps and status indicators (shall) be exercised including the authority for application..."
Paragraph 14.4.2 of Section QA-14 of W-H Quality Assurance Manual states that: "The status of materials, parts, systems, and components will be identified through the use of inspection reports and tags."
Contrary to the above, as of April 28, 1982, no effective method was in use by W-H Constructors to indicate expansion anchor bolt torquing inspection status.
This is a Severity Level IV Violation (Supplement II).
II. 10 CFR 50, Appendix B, Criterion V states, in part, that: " Activities affect-ing quality shall be prescribed by documented instructions, procedures, or drawings...and shall be accomplished in accordance with these instructions, l
procedures, or drawings."

Section 16.4.5 of Chapter 16 of the PSAR states, in part, that:
... quality I
related activities are documented and controlled by written procedures and instructions."
i paragraph 3.2 of Bechtel Specification G-002(Q), Rev. 6, which is included l
as part of W-H Constructor's contract document, states that deviations from l
specification requirements must be identified and dispositioned.

pany is hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including:
(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affimation. Where good cause is shown, consideration will be given to extending your response time.
