s-Appendix A NOTICE OF VIOLATION Arizona Public Service Coupany Docket No. 50-528 P. O. Box 21666 Construction Permit No. 141 Phoenix, Arizona 85036 As a result of the inspection conducted on March 22 - April 30, 1982, and in accordance with URC Enforcement Policy, 10 CFR Part 2, Appendix C. 47 FR 9987, March 9, 1982, the following violations were identified:
1.
Criterion II of Appendix B to 10 CFR 50 states, in part, that,
Activities affecting quality shall be accomplished under suitable controlled conditions.
Controlled conditions in-clude... suitable environmental conditions for accomplishing the activity, such as adequate cleanness...."
bechtel Procedure UP/P-QCI No. 13, "llousekeeping" states in paragraph 6.4.4.1, "A Lone IV area or itaa shall be swept clean of dirt and debris...." and states in paragraph 6.5.2.2 for a Zone V area, " Garbage, trash, scrap, litter, and other excess waste materials shall be collected and disposed of by burning, burying or removal from the jobsite.
Such excess material chall not be allowed to accumulate."
Contrary to the above on April 15, 1982, the inspector observed the following:
a.
Excessive debris, concrete dust and fire protective insulation fines were found accumulated on equipment and floors in the Zone IV Class IE battery charger rooms.
Several equipment protective coverings had been removed exposing opened electrical cabinets which were energized to the degraded environment.
b.
Empty soft drink cans, insulation pads, hoses and other debris had been discarded in a Zone V confined area be-tween the outside control room shutdown cabinets.
An accumulation of concrete and fire protective insulation fines had also accumulated within the cabinets which were posted as energized.
This is a Level V Violation (Supplement II) 2.
Crition V of Appendix B to 10 CFR 50 states, in part, thae.
" Activities affecting quality shall be prescribed by docarented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instruction procedures, or drawings...."

1 The Eechtel Administrative Control Procedure AD 115, " Temporary i
l Modification Controls," states in. paragraph 5.1.2, " Field identification of temporary modifications shall be accomplished by attaching-the Temporary Modification Tag.... Tags shall be placed conspicuously and where ever.possible'directly on the 3
wire, cable or component actually modified."
Contrary to the above on March 26 and 29, 1962, the inspector
'noted that pressure safety valves had been replaced with temporary vents at the following Unit 1 locations and the modified components were not tagged as required =by the pro-cedure.

Pursuant to the provisions of 10 CFR 2.201, Arizona Public Service Company is hereby required to submit to this office within thirty days of the date of this Notice, a written statement or. explanation in reply, including:
(1) the corrective steps which have been taken and the results achieved:
(2) corrective steps which will
.be taken to avoid further items of noncompliance; and (3) the date when full compliance will be achieved.
Consideration may'be given to extending.your response time for good cause shown.
