Dear Mr. Keppler:

On April 23, 1982, Mr. S. J. Brewer of Public Service Company of Indiana, Inc.

(PSI) notified your of fice of a potentially reportable item as required by 10 CFR 50.55(e) . This item pertained to the discovery of spalling at one location on the underside of the 401 ft. elevation floor slab in the Unit 2 Auxiliary Building. Examination of the affected surface indicated the spalling was due to aggregate shear failure and not unsound concrete.

The probable cause of the concrete spalling was the movement of a 4100 Manitowac crane over tSe affected area. Calculations by PSI's architect -

engineer, Sargent & Lundy indicate that while the force exerted by the crane was great enough to spall the concrete, stress in the reinforcing steel was not sufficient to damage the steel. Any remaining spalled or cracked concrete will be removed from this area and an inspection of the reinforcing steel will be made to ensure its integrity has not been impaired. The method of repair of this area will be determined subsequent to this inspection.

This letter is intended to fulfill the requirements of an interim report as defined in 10 CFR 50.55(e). A further report will be filed on or before July 23, 1982. If you have any questions on this report, please feel free to contact me.
