Dear Mr. Keppler:
10 CFR Part 21 Defect General Electric USPS Load Driver Connection Plugs Docket No. 50-461/CPPR-137 On December 8, 1981, Mr. A.J. Budnick of Illinois Power notified Mr. H.M. Wescott, U.S. NRC Region III, of a potential reportable defect per 10 CFR Part 21 regarding certain NSPS Load Driver Connection Plugs supplied by General Electric Company.
Based upon an evaluation, Illinois Power is notifying the Commission of a 10 CFR Part 21 defect.
This report provides information as required by 10 CFR Part 21.21(6)(3).
(i)
A.J.
Budnick, Director-Quality Assurance informed the NRC of a potential defect on December 8, 1981.
Mr. U.C.
Gerstner, Executive Vice President, by means of this report, hereby informs the Commission of a 10 CFR Part 21 defect.
(ii)
The components involved are load driver printed circuit board connection plugs (parts list item #695) in NSPS panels P661, P662, P663 and P664 on elevation 800 in the main control room.
(iii)
Supplied by - General Electric Company, San Jose, CA.
Manufactured by - General Electric Company, Daytona Beach, FL.
(iv)
The defect is that each connection plug has 4 jumpers missing and the resultant safety hazard is that the load drivers cannot perform their safety function of energizing or de-energizing their loads.

On December 1, 1981, Maintenance Work Request #A0433 was completed which verified that the subject connection plugs were missing the 4 jumpers.
(vi)
A total of 101 connection plugs are missing 4 jumpers each.
These plugs are located in elevation 800' main control room NSPS panels as follows:
P661 45 Connection Plugs P662 39 Connection Plugs P663 11 Connection Plugs P664 6 Connection Plugs TOT-(vii)
A maintenance work request (2163) was issued by Illinois Power Clinton Power Station maintenance organization to correct the connection plug defect.
Completion of this work is pending delivery of correct jumper material.
(viii)
General Electric (CPS site) was alerted by General Electric (San Jose) that the subject jumpers possibly did not exist.
General Electric (site) alerted Illinois Power who in turn generated the Maintenance Uork Request which verified the missing jumpers.
Illinois Power repeats all performance tests which GE performed at the factory on the NSPS panels and therefore, this problem would have been discovered by Illinois Power during checkout and testing.
