1 APPENDIX A LOTICE OF VIOLATION Kashington Public Power Supply System Docket No. 50-460 P. O. Box 966 Construction Permit Richland, Washington 99352 No. CPPR-134 As a result of the inspection conducted on March 1-31, 1982 and in accordance with the URC Enforcement Policy, 10 CFR Part 2 Appendix C, 47 FR 9887 (March 9, 1982), the following violation was identified:

10 CFR 50 Appendix B Criterion V, as addressed in paragraph 17.4.5 of the Quality Assurance Program in Section 17 of the PSA2, states in part:

" Activities affecting quality shall be prescribed by doc-unented instructions, procedures, or drawings of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings..."

The J. A. Jones Construction Company ASME Quality Assurance Manual Part XIV and implementing procedure POP-703W require that when a nonconforming condition is identified, the quality verification inspector affix a hold tag to the nonconforming item or system and record the hold tag number on the associated traveler or process control documentation.

Contrary to the above, on March 9, 1982 the quality verification organication had not yet placed the hold tag nor affixed the tag number to process control documentation for reactor coolant system field weld Eb-016, for a nonconforming condition identified on March 2, 1982.

As a result, post-weld heat treatment of the weld cctnenced and continued until the weld reached 900 degrees, at which time the existence of the nonconforming condition was identified and a risk decision was made to proceed without prior resolution of the nonconformance.

The nonconformance involved lach of documen-tation demonstrating that tri-sodium-phosphate cleaner had been removed from the weld area prior to its having been welded on August 17, 1981.

1his is a Severity Level V Violation (Supplement II), applicable to Unit 1.
