APPENDIX A NOTICE-0F VIOLATION Northeast Nuclear Energy Company Docket No. 50-423 Millstone Nuclear Power Station, Unit 3 License No. CPPR-113 As a result of the inspection conducted March 1-31 and April 1-9, 1982, and in accordance with the NRC Enforcement Policy, 47 FR 9987 (March 9, 1982), the following violations were identified:

A.

10 CFR 50, Appendix B, Criterion y and Northeast Utilities QA Program Topical Report,'Section 5, states that activities affecting quality shall be accomplished in accordance with (the prescribed) instructions, proce-dures, or drawings.

Stone and Webster Specification No. 2200,000-914, for Mechanical Equip-ment Erection, Section 3.2, states that all installation and erection work shall be perfonned in strict conformance with applicable codes, standards, and regulations, the manufacturer's approved instructions and reconTnenda-tions, and with the engineer's approved specifications, drawings and direc-tives.

Contrary to the above, on March 10, 1982, the RHR Pump Alignment Plates were being leveled by grinding the support plate which is contrary to the requirements specified in the Design Drawing EV-52A, Revision 5, and its associated design change E&DCR Number F-J-6640.

Furthermore, during this grinding, the support plate thickness was reduced below the minimum of 1.537 inches allowed by AS'M A6, This is a Severity Level V Violation (Supplement II).

B.

10 CFR 50, Appendix B, Criterion II and Norhteast Utilities QA Program Topical Report, Section 2, states that the QA Program shall provide for indoctrination and training of personnel performing activities affecting quality as necessary to assure that suitable proficiency is achieved and maintained.

Stone & Webster Corporate Procedure QS-2.12, Indoctrination and Continuing Education, Revision 8, Section 7.1, states that the QA Department shall provide indoctrination and continuing education (CE) of personnel in accord-ance with Quality Assurance Department Procedures (QAD).

QAD Procedure No. 4.2.3 specifies that the supervisor of Field Quality Control shall implement the Continuing Education program by coordinating necessary activities with System Services.

Contrary to the above, as of April 1,1982, the Stone and Webster Field QC Department was not providing indoctrination and continuing education of personnel in accordance with Quality Assurance Procedure No. 2.1, QA Department Continuing Education System Revision A, issued on September 6, 1978. The following are examples:

Section 5.3 requires that approved CE courses shall be presented in accordance with the monthly forecast, unless changed in writing. Con-trary to this, a total of six CE courses scheduled during the months of October through December,1981, and January through February,1982, were neither held nor deleted or rescheduled.

Section 5.4.1 states that copies of attendance reports shall be kept at the CE presentation location. Contrary to this, three courses were held at the site and the attendance reports were not available.

This is a Severity Level V Violation (Supplement II).

Pursuant to the provisions of 10 CFR 2.201, Northeast Nuclear Energy Company is hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including:

(1) the correc-tive steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full com-pliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affinna-tion. Where good cause is shown, consideration will be given to extending your response time.

The responses directed by this Notice are not subject to the clearance procedures of the Office of Management and Budget as required by the Paperwork Reduction Act of 1980, PL 96-511.
