Appendix NOTICE OF VIOLATION Genesee Memorial Hospital License No. 21-15131-01 As a result of the inspection conducted on March 25, 1982, and in acccrdance with the NRC Enforcement Policy, 47 FR 9987 (March 9, 1982), the following violations were identified:
1.
License Condition No. 14 requires that licensed material be possessed and used in accordance with the statements, representations, and pro-cedures contained in certain referenced applications and letters.
The referenced letter dated November 21, 1977, states in Item 2, the procedures described in Appendix B, Section 2 of the Medical Licensing Guide will be followed. Appendix B states the Medical Isotope Committee shall meet at least once in each calendar quarter.
Contrary to the above requirement, you failed to hold quarterly meetings.
Specifically, the Medical Isotope Committee met once in 1979, once in 1980, twica in 1981, and on April 4, 1982.
This is a Severity Level V violation (Supplement VI).
This is a repeat item of noncompliance.

License Condition No. 14 requires that licensed material be possessed and used in accordance with the statements, representations, and pro-cedures contained in certain referenced applications and letters.
The referenced letter dated October 10, 1977, states in Item 11 the procedures described in Appendix D, of the Medical Licensing Guide will be followed for calibration of survey instrument.
Appendix D states that calibrated survey instruments are acceptable when the measured exposure rate differs from the true exposure rate by i20%.
Contrary to the above requirement, your Victoreen 498 survey meter calibrated on February 24, 1982, was reading 28% less than the trae exposure rate.
This is a Severity Level V violation (Supplement VI).
Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written state-ment or explanation in reply, including for each item of noncompliance:

Appendix 2
(1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full com-pliance will be achieved. Consideration may be given to extending your response time for good cause shown.
