,f Stone & Webster Engineering Corporation (SWEC), pursuant t l's ions of 10CFR21, hereby provides notification of a " defect" concerning the ?!aine Yankee Atomic Power Plant, owned by the t!aine Yankee Atomic Power Company and for which SWEC provided both engineering and construction services.

In accordance with the reporting requirements of Section 21.21(b)(3), the following information is submitted:

Identification of the facility, or the basic component supplied for such facility, which contains a defect:

Nature of the defect and the safety hazard which could be created by such defect:

There exists a departure from the technical requirements of a procurement document in that the Safety Injection Actuation System could fail to operate as required.

The SIAS design is a basic component and has been delivered to a purchaser.

A single relay provides the SIAS actuation signal due to low pressurizer pressure. This relay is controlled by a 2 out of 4 logic composed of four independent pressurizer pressure signals. Failure of this relay or loss of power to this relay will prevent automatic actuation of safety injection for those accidents resulting in loss of reactor coolant system pressure but not high containment pressure.

Safety injection is necessary to mitigate the consequences of these accidents. Lack of automatic actuation of safety injection represents a major reduction in the degree of protection provided to the public health and safety and results in a substantial safety hazard.

We have been informed that the NRC is aware of this problem per telephone call between Nat Villalva (NRC) and R. B. Bradbury (SWEC) on February 1, 1982.

In addition, IE has issued PNO-1-82-13 on this subject.
