e Appendix NOTICE OF VIOLATION Consumers Power Company Docket No. 50-155 As a result of the inspection conducted on March 15-18, 1982, and in accordance with the NRC Enforcement Policy, (10 CFR Part 2, Appendix C),
47 FR 9987 (March 9, 1982), the following violations were identified:
1.
10 CFR 20.203(c)(1) and (2) states that each high radiation area shall be conspicuously posted and each entrance or access point to a high radiation area shall be equipped with a control device, alarm, or maintained locked.
Contrary to the above, a high radiation area next to the fuel pool surge tank measuring 350 mR/hr was not posted or controlled as a high radiation area.
This is a Severity Level IV violation (Supplement IV).

Technical Specification 6.11 states, in part, that radiation protec-tion procedures shall be adhered to for all operations involving radiation exposure. The following instances of failure to meet this requirement were identified during this inspection.
a.
Big Rock Point Manual, Chapter 11.6.5, prohibits smoking in controlled areas.
Centrary to the above, the inspector observed several workers smoking while cleaning up a posted contaminated area.
b.
Big Rock Point Manual, Chapter 11.9.1, states that protective clothing is to be removed while stepping out of a contaminated area.
Contrary to the above, the inspector observed several workers violate step-off boundaries by stepping back into the contaminated area after removing their protective clothing or by stepping out of the contaminated area without first removing their protective clothing.
c.
Big Rock Point Manual, Chapter 11.6.8, states that persons entering a boundaried area must be RWP-exempt, obtain an RWP, or have C&RP coverage.

Contrary to the above, a group of workers (not RWP-exempt) per-formed work in the boundaried area of the radwaste room for a period of at least two days without an RWP or C&RP technician coverage.
d.
Big Rock Point Manual, Chapter 11.11.1.7, states in part that high radiation areas will have a locked access.
Contrarf to the above, a decontamination crew left a high radiation area chain unlocked and unattended.
e.
Big Rock Point Manual, Chapter 11.9.5, states in part that all vacuum cleaners used in posted areas have covers with locks.
Contrary to the above, on March 19, 1982, an unlocked vacuum cleaner was used in the posted area near the main condensers.
This is a Severity Level IV violation (Supplement IV).
Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written state-ment or explanation in reply, including for each item of noncompliance:
(1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further nonccepliance; and (3) the date when full com-pliance will be achieved. Consiueration may be given to extending your response time for good cause shown.
