A APPENDIX A Combustion Engineering, Inc.
Power Systems Group Docket No. 99900401/82-01 NOTICE OF NONCONFORMANCE Based on the results of an NRC inspection conducted on March 22-26, 1982, it appears that certain of your activities were not conducted in accordance with commitments.
Section 17.5 of CENPD-210-A requires that activities affecting quality shall be prescribed by documented instructions, procedures, or_ drawings and shall be accomplished in accordance with these instructions, procedures or drawings.
Section 2.2 of Quality Assurance of Design Procedure (QADP) 5.3 (Interface Control) Revision 1, dated May 1, 1980, states in part, "... review, concurrence, approval, etc., of design information (i.e., specifications, calculations, drawings, etc.) shall contain a Document Distribution / Approval form.
The completed Document Distribution / Approval form shall be returned to the originator and made a quality record."
In addition, Section 6.4.2 of Plant Engineering QA Procedures.(PEQAP) No. 001 (Design Quality Assurance Procedure) Revision 1, dated August 18, 1975, states in part, "... Response to interface document comments will reference the document containing the comments and will be forwarded with a document approval checklist or equiva-lent. All document app. oval checklists will be returned to Plant Engineering and will be filed with a copy of the transmittal letter."

Contrary to the above,'the_following specifications did not have the required documentary evidence of interface reviews on file:
