Event
Description:
An administrative review of a water level anomaly, which oc-curred during reactor startup following the Refuel V Outaae determined that PNPS entered an LC0 (Table 3.1.1 Note 1.A) without taking the appropriate action which was 'to initiate insertion of control rods.
Cause and Corrective Action: A reactor shutdown and a half scram which were initiated as a result of this anomaly were inapproate for addressing the RPS instrumentation.
Special Order 82-04 is being developed to prevent recurrence of this event and will be issued on April 28, 1982.
