APPENDIX A HOTICE OF VIOLATION Public Service Electric and Gas Company Docket No. 50-354 License No. CPPR 120 Hope Creek, Unit 1 During inspection on March 1 - April 4, 1982, the following violation was identi-fied under the NRC Enforcement Policy,10 CFR 2, Appendix C (47FR9987, March 9, 1982):
10 CFR 50, Appendix B, Criterion V, states, in part, that:
" Activities affecting quality shall be prescribed by documented instructions, pro-cedures, or drawings... and shall be accomplished in accordance with these instructions, procedures, or drawings."
Section 16.4.5 of Chapter 16 of the PSAR states, in part, that:
... quality related activities are documented and controlled by written procedures and instructions."
Paragraphs 7.1, 7.2, and 8.4 of Bechtel Specific Work Plan / Procedure SWP/P-19 state that issuance of filler metal is to be controlled for each weld.
Contrary to the above, as of March 11, 1982, electrical penetration W101D was tack welded into position prior to issuance of filler metal for the weld joint.
This is a Severity Level V Violation (Supplement II).
Pursuant to the provisions of 10 CFR 2.201, Public Service Electric and Gas Company is hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including:
(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under an oath or affirmation.
Where good cause is shown, consideration will be given to extending your reeponse time.
