APPENDIX A NOTICE OF VIOLATION Mahe Yankee Atomic Power Company Docket No. 50-309 Maine Yankee Atomic Power Station License No. DPR-36 As a result of an inspection conducted at the Maine Yankee Atomic Power Station.
on February 9 - March 22, 1982 and in accordance with Enforcement Policy, 47 FR 9987 (March 9,1982), the following violation was identified:
10 CFR 50, Appendix B, Criterion III, requires desigri control measures to be implemented for field changes.
The Maine Yankee Operational Quality
. Assurance Program as implemented by Procedure 0-01-03, Installation /
Fabrication Instructions, Revision 0, requires changes to install.ation instructions for safety related modifications to be reviewed and approved by the same organizations that approved the original instructions.
Contrary to the above, on March 8, 1982; the installation instructions for Engineering Design Change Request 80-45, Change 3, Time Delay Feature for Secondary Component Cooling Pumps, were changed without review and approval from an independent engineer or the quality assurance departraent, both of whom had approved the original instructions.
This is a Severity Level V violation (Supplement 1.E).
Pursuant to the provisic,qs of 10 CFR 2.201, Maine Yankee Atomic Power Company is hereby required to sul mit to this office within thirty days of the date of this Notice, a written s:atement or explanation in reply, including:
(1) the corrective steps which tave been taken and the results achieved; (2) corrective steps which will'be taken to avoid further violations; and (3) the date when full compliance will be achieved.
Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation. Where good cause is shown, consideration will be given to extending your response time.
