r APPENDIX A NOTICE OF VIOLATION Niagara Mohawk Power Corporation Docket No. 50-220 Nine Mile Point. Unit 1 License No. DPR-63 As a result of the inspection conducted on March 1 to April 3,1982, and in accordance with the NRC Enforcement policy, (10 CFR 2, Appendix C) published in the Federal Register Notice (47 FR 9987) dated March 9, 1982, the following violations were identified:

A.

Technical Specification 6.8.1 states in part:

" Written procedures and administrative policies shall be established, implemented, and maintained..."

Laboratory Instrument Procedure No. IV.A.20, " Operation of the L&N Model 4866 Conductivity Monitor," Revision 1, dated February 3, 1981 requires in section 4.3.2 that the calibration check of the conductivity monitor be performed using three resistors from a list of a 10 megaohm, 1 megaohm, 100 kilo ohm, 10 kilo ohm and 1 kilo ohm resistors. The data sheet for the calibration check also requires supervisory review of the completed form.

Contrary to the above, on March 4, 1982, the supervisory review of the monthly calibration check data sheets from February 1981 to March 1982 had not been performed and the resistors used for the calibration check during the same time period, were not of the specified resistance.

This is a Severity Level V Violation (Supplement I).

B.

Technical Specification 6.11 states:

" Procedures for personnel radiation protection shall be prepared consistent with the requirements of 10 CFR Part 20 and shall be approved, maintained, and adhered to for all operations involving personnel radiation exposure."

Radiation Protection Procedure RP-2, " Radiation Work Permit Procedure,"

Revision 1, dated February 13, 1979 states in Section 5.4:

"The leadman is responsible for familiarizing personnel with all instructions on the permit, and insuring that these instructions are strictly followed."

Radiation Work Permit (RWP) #7403 dated April 2,1982, Special Instructions section stated:

" Wear dosimetry on head while working under vessel."

Contrary to the above, on April 2, 1982, four individuals, including one designated as the leadman, working under the reactor vessel were found to be wearing their dosimetry on their chests instead of their heads.

This is a Severity Level V Violation (Supplement IV).

Pursuant to the provisions of 10 CFR 2.201, Niagara Mohawk Power Corporation is hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, includin (1) the corrective steps which have been taken and the results achieved; (g:2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation. Where good cause is shown, consideration will be given to extending your response time.
