APPENDIX A NOTICE OF VIOLATION Niagara Mohawk Power Corporation Docket No. 50-410 Nine Mile Point, Unit 2 License No. CPPR-112 As a result of the inspection conducted March 1-26, 1982, and in accordance with NRC Enforcement Policy, 47 FR 9987 (March 9,1982), the following violation was identified:

10 CFR 50, Appendix B, Criterion V states in part, " Activities affecting quality shall be prescribed by documented instructions... and shall be accomplished in accordance with these instructions...."

The Nine Mile Point Nuclear Station, Unit 2, PSAR, Appendix D adopts installation specifications.

Installation specification P30lV for Reactor Controls, Inc. requires compliance with General Electric Field Disposition Instruction FDI 15/31263-2.

General Electric Field Disposition Instruction, FDI-15/31263-2, RPV Recirculation Nozzle Modifications, states in part, in Section 4.9.2, that data sheets shall record the location and description of all blocking devices used and the data sheet shall also record the type of measuring device used and its expected accuracy for the purpose of tolerance adjustments.

Contrary to the above, Reactor Controls, Inc. had not recorded on data sheets the following factual information:

1.

Measurements of the location of the blocking devices.

2.

Measuring device used for determining the location of the ten jet pump risers prior to restraining.

3.

Measuring device used concerning dimensional tolerance verification of weld prep nozzles #K-1241 and #K-1246.

This is a Severity Level V Violation (Supplement II).

Pursuant to the provisions of 10 CFR 2.201, Niagara Mohawk Power Corporation is hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including:

(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved.

Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.

Where good cause is shown, consideration will be given to extending 'your response time.
