The initial post-accident attempt to enter the TMI-2 Reactor Build g

May of 1930 was aborted due to an airlock door malfunction. Since t time, several reactor building operations have been delayed or cancelled due to malfunctioning doors (October 16,1980; June 25,1981; July 20,1981; March 19,1982; April 14,1982). A lack of understanding of airlock design, the status of airlock modifications, and inadequate personnel training may have added to these occurrences.

Our concern in this area is based on:

)

(1) the potential for future unnecessary exposures /overexposures inside the reactor building, (2) potential inability.to ingress / egress the reactor building during an emergency, and (3) potential personnel injury while operating tha doors ( folent opening of door due to possible unequalized pressure).

In resolving our concerns, please respond to the following questions:

Is the current design / condition of the airlocks adequate to support safe operations inside the reactor building?

b.

What steps have bien taken,to correct the airlock malfunctions which were identified during the post-accident entries?

What steps have ticen taken to ensure that personnel involved c.

with reactor building entries are thoroughly familiar with airlock design, operations, and hazards?

d.

What routine survelilances could be performed to assure.that the alternate equipment hatch airlock is operable.
