Dear Sir:

The NRC was notified of a potentially reportable deficiency in the referenced telephone conversation. At that time, it was estimated that a determination of reportability would be made within thirty (30) days.

Due to additional investigation and corrective action required, an Interim Report is attached.

It is now expected that this information will be finalized by May 26, 1982, at which time a complete report will be submitted.

INTERIM REPORT - DER 82-4 REPORTABLE DEFICIENCY 50.55(e)

PVNGS UNITS 1, 2 AND 3 1.

Description of Deficiency During a review of the safety-related instrument tubing. clamps, it was found that locking devices were not installed on the threaded fasteners. The drawings contained in the Design Specification illustrate a typical fastener using a single ASME III NF nut.

This was not sufficient information for Construction to comply with ASME III NF4725 which requires threaded fasteners to be provided with locking devices such as jam nuts, stop nuts, or drilled and wired nuts.

This condition is evaluated as Reportable under the requirements of 10CFR50.55(e).

If the instrument tubing were to become dis-engaged from the seismic supports, damage to the tubing could render the safety-related instrumentation inoperative.

II.

Approach To and Status Of Proposed Resolution Design Change Packages will be issued to have all the installed clamps corrected to comply with the Code, and Specification 13-JM-715 will be clarified to provide definitive instructions concerning the threaded fasteners.

III. Projected Completion of Corrective Action and Submittal of the Final Report Evaluation of this condition and subnittal of the Final Report is forecast to be completed by May 26, 1982.
