Subject:

Nuclear Project No. 2 l

10CFR50.55(e) Reportable Conditions #10, 58, 62, and 83 Deficiencies in Concrete Expansion Anchor Program, and 10CFR50.55(e) Reportable Condition #73 Deficiencies in Installed Grout.

Reference:

Expansion Anchor Deficiencies:

WPPSS Ltr. No. G02-79-120 dated July 2,1979 WPPSS Ltr. No. G02-79-138 dated August 7, 1979 WPPSS Ltr. No. G02-81-245 dated August 18, 1981 Grout Deficiencies:

l WPPSS Ltr. No. G02-80-302 dated December 15, 1980 WPPSS Ltr No. G02-82-143 dated February 5, 1982 The referenced letters were presented as final reports of the subject reportable conditions.

Recent NRC onsite inspection of activities associated with these reports show them to be of insufficient detail to permit NRC analysis and evaluation of the identified deficiencies and the appropriateness of your corrective actions. 10CFR50.55(e) requires that "...the mport shall include a description of the deficiency, an analysis of the safety implications and the corrective actions taken, and sufficient infomation to permit analysis and evaluation of the deficiency and of the corrective action..."

l NRC onsite inspections also indicate that a complete review of corrective actions taken by all contractors to resolve past deficiencies in concrete expansion anchor and structurally significant grout installation b warranted. The scope of this review should not be limited to activities associated with safety related pipe hanger installation alone, but should include all safety related hangers, equipment or components which rely on these materials for structural support.
