APPENDIX A NOTICE OF VIOLATION South Carolina Electric & Gas Docket No. 50-395 V. C. Summer License No. CPPR-94 As a result of the inspection conducted on February 1-28, 1982, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1980), the following violation was identified.
10 CFR 50, Appendix B, Criterion V, requires activities affecting quality be prescribed by documented instructions, procedures or drawings appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures or drawings.
Section 6.1.(g) of Startup Manual Procedure SUM-B-13 "Startup Field Reports (SFR)" states that the Lead System Supervisor will implement the SFR in accordance with the approved disposition of the SFR. The approved disposition f SFR 2198 required valves 3129 A and D to be flow tested.
Contrary to the above, SFR 2198 was not implemented in accordance with the approved disposition in that valves 3129 B and C were flow tested instead of valves 3129 A and D.
This is a Severity Level V Violation (Supplement I.).
