Dear Mr. Keppler:
This letter serves as our final report pursuant to 10CFR50.55(e) concerning the masking of certain weld indications during Unit 1 Suppression Pool Clad Floor Plate installation activities.
Mr. L. McGregor of your office was first notified on November 12, 1981, by Messrs. E. Riley and G. Leidich that The Cleveland Electric Illuminating Company was investigating this deficiency.
Subsequently, on November 18, 1981, Newport News Industrial Corporation of Ohio (NNICO) filed a 10CFR21 report with your office and on December 11, 1981, we filed an interim report pursuant to 10CFR50.55(e).
Our report includes a description of the deficiency, an analysis of safety implications, and information concerning corrective actions taken since the interim report.
Description of Deficiency l
NNICO Quality Assurance personnel received information on November 10, 1981, l
that their craf t welding personnel had been int,tructed and were performing welding operations to mask indications from detection during inspection.
Speelfically, this concerned indications created during semi-automatic pro-l duction welding related to installation of some Unit 1 Suppression Pool Floor Plates.
l NNICO immediately stopped all work in the affected area and conducted an investigation to establish the validity and extent of the deficiency.

Mr. Janes G. Keppler March 31, 1982 Analysis of Safety Implications The presence of these indications in the welds could possibly have contributed to the failure of the floor plates during an accident condition, thus resulting in potential containment breach and potential off-site exposure exceeding allowable limits.
Corrective Action As previously outlined, NNICO personnel directly associated with the suspect weld joints have been reassigned to other work areas or removed from the job site.
Additionally, NNICO Welding Engineering personnel have conducted indoctrination / training sessions concerning techniques to be employed for subsequent welding operations in the suppression pool area for all personnel, including supervision currently assigned to this work area.
Since subnittal of the Interim Report, NNICO has completed their investigation and in accordance with NR 17-316 (Rev. O and Rev. 1) all questionabic weld metal applied to mask indications has been removed and all related excavations have been satisfactorily inspected to insure complete removal of all masked indications associated with this deficiency.
The replacement of weld metal detailed in the disposition of NR 17-316 is currently on hold because NR 17-334 was initiated by NNICO on January 20, 1982, to identify other defects related to the welding of these floor plates.
Upon resolution of NR 17-334, this portion of the disposition of NR 17-316 will be completed.
