To date, we have only received fees and fee fonns for Haddam fleck and tillistone 1 and 2.

Please provide copies of the requests, preliminary or final fee form (whichever is appropriate) and a copy of the approval (if issued) for all licensees who have made such a request. We have seen RIDS and/or menos on the following cases:
