PVNGS UNIT 1 I. Description of Deficiency During an internal cleanliness inspection of Steam Generator

  1. 2, e linear indication in the nozzle outlet (Bechtel Weld W-001) was discovered by visual inspection. The indication is approximately b" in length and appears to run parallel with the root bead in the root and heat affected zone. Prior to the visual discovery, the following NDE's had been previously completed as satisfactory on the subject weld.

o RT Inspected to ASME III, August 13, 1980 o PSE UT Test Per ASME XI, August 20, 1980 o PWHT Completed, September 24, 1980 II. Analysis of Safety Implications The linear indication was removed by grinding. The area was visually inspected, and a surface examination (PT) was performed.

UT thickness measurement to verify minimum wall was performed and the minimum thickness obtained was 1.56", which is greater than the minimum wall requirements of 1.50". No welding was required, Radiography was performed for information only. All examinations are acceptable; therefore, this condition is evalu-ated as Not Reportable since if left undetected and uncorrected, it would not have represented a safety significant condition.

III. Corrective Action Nonconformance Report WC-502 will be dispositioned as "Use-As-Is" and will have the reports identified in II, included as attach-ments,
