ZZ ZZZZZZ Unit removed from service for a planned outage to perform ice con-denser surveillance tests, miscel-laneous maintenance items and inspect the steam generator tube lane blocking devices.
The steam generator inspec-tion showed damaged tubes in No. 11 and No.14 steam generators of suffi-cient magnitude to warrant tube plugging.
Five tubes on each end in both steam generators were plugged.
The unit remained out of service at the end of the month.

The Unit entered this reporting period at 100% Reactor power and operated at 90% to 100% until December 24, 1980 when a reduction was started to remove the Unit for a planned out-age. The Unit was removed from service 1454 hours0.0168 days
0.404 hours
0.0024 weeks
5.53247e-4 months
, December 24, 1980.
While degassing the Reactor Coolant System, RMS Channel 26 (Aux. Vent Monitor) showed an increase each time the VCT was vented to the Waste Gas System. These releases were quanti-fied.
It was found later to be a valve packing leak on RRV-300,, pressure regulator from the VCT.
Unit entered Mode 5 at 2345 hours0.0271 days
0.651 hours
0.00388 weeks
8.922725e-4 months
, December 25, 1980 for the planned outage to inspect Ice Condenser doors.
Unit remained in Mode 5 for the remainder of this reporting period.
Total electrical generation for the month was 614,070 mwh.

DOCKET NO.
50 - 315 UNIT NAME D. C. Cook - Unit No.1 DATE l-14-81 COMPLETED BY B. A. Svensson TELEPHONE (616) 465-5901 PAGE 1 of 1 MAJOR SAFETY-RELATED MAINTENANCE DECEMBER, 1980 M-1 The north waste gas compressor suction valve, RRV-378 had a body-to-bonnet leak.
Disassembled valve and replaced the valve diaphragm.
Reassembled and had valve tested.
M-2 Glycol return containment isolation valve, VCR-20 failed to close during a test to determine the cause of a previous failure.
The valve was removed from service, the lower stem bushing was honed, all parts were cleaned and the valve diaphragm was replaced.
The valve was then tested satisfactorily.
M-3 No. 4 steam generator stop valve dump ' valve, MRV-241 was leaking by.
Replaced the stem, plug, seat ring and gaskets.
Valve tested satisfactorily.
M-4 No. 3 steam generator blowdown regulating valve, DRV-331 had an air leak at the operator.
Replaced the operator diaphragm and a gasket.
Valve tested satisfactorily.
M-5 Packing leak on the south safety injection pump discharge isolation, SI-lllS.
Valve was repacked.
M-6 Isolation valve, MPP-212V1 for pressure transmitter, MPP-212, up-stream of #1 SG stop valve had packing leak.
Valve was repacked.
C&I-l The rod position indication system for rods D4, D12 and M12 indicated a position greater than 13 steps from group demand. The primary and secondary coil voltages were measured and found to be within specifi-cations.
The panel meter circuit was adjusted to provide proper in-dication.
C&I-2 Radiation monitoring system channel R33, gland steam condenser vent monitor failed to the low end of scale.
The problem was traced to the junction box containing the detector electronics. As the door of the box was closed, the circuit board would ground to the check source motor circuit.
The check source motor circuit leads and the circuit board edge were taped to prevent electrical contact.
The channel was returned to service.
C&I-3 RCS Loop 3, CVCS letdown air operated valve, QRV-161 had failed open.
A stuck solenoid valve was freed and the valve was cycled several times to check its operation.
