A completed, but unsigned copy of DOE /NRC Form Number 741 is enclosed.

Your comments concerning its completeness and correctness will be appreciated.

As I mentioned on the telephone, we would like. Exxon, if possible~ to attempt ,

to determine the serial numbers of the individual fuel plates in our shipment.

Apparently some early modifications of the FHU's led to a4erial number mix-up.

We, and the NRC, would like to remove the unknown serial numbers from our inventory 1.is t.

We are still trying for June 21 'as the shipping'date from UCLA.

I. Org anization Figure 1 illustrates the relationship between the various working groups and.the license-holder (the Board of Regents). All official communi cations involving licensing that originate at UCLA, are to bear the nignature of f

Vice-Chancellor Hobson.' The point of contact at UCLA is Dr. Walter Wegst, Director, Research and Occupational Safety. The Radiatidn Safety. Officer for the program is John C. Evraets, who is responsible f'or the Quality Assurance Program and its implementation (10.CFR.71.12).

The Nuclear Energy Laboratory has accepted responsibility for making the shipping arrangements, and for supplying the labor necessary to trans-port the fuel from the reactor rocm to the cask. provida the labor and direction-as-necessary (via Mr. Ostrander) under the surveillance of Ibn Hornor, Resident Health Physicist (as deputy for the Radiation Safety Officer) . During this unique operation, Mr. Evraets will

assign Dr. John Kaufman (Environmental Health and Safety) to support Ik. Hornor.

Tee shipping arrangements, under the redponsibility of Mr. Ashbaugh, will ir lude prior notification o'f appropriate agencies and interests including (a) the Los Angeles Police Department; (b) the FBI; (c) the California Highway Patrol; (d) County and/or State jurisdicticns along the transit path from UCLA to EX70N Nuclear, Idaho Falls, Idaho.

The UCLA Risk Management Coordinator shall be informed of these arrange-r.ents, and shall have authority (via the Purchasing Department) to approve the final arrange =ents.
